General Description:

This position assists students with disabilities to prepare for the workforce as they transition from high school to post-high school life.

Qualifications:
Knowledge, Skills, Abilities Required:

  • High school diploma or equivalent.

  • To perform this job successfully, an individual should be proficient with e-mail, internet software and word processing software.

  • Certificates as determined by the District including a valid type 10 driver's license.

  • Bilingual Preferred

Essential Functions include the following:

  • Participates in activities related to the recruitment of potential clients.

  • Assists the YTP team in determining VR eligibility of applicants.

  • Assists students in acquiring necessary mobility to keep appointments.

  • Implements a system for disseminating YTP information to employers.

  • Analyzes work sites and matches YTP clients to work sites.

  • Maintains and updates a system for tracking active and inactive employers.

  • Provides structured training at community job sites consistent with each client's targeted vocational goals.

  • Provides follow-up support to students.

  • Collects data as necessary to document students' progress on the job.

  • Maintains an accurate case file on each YTP client.

  • Participates in all team meetings as invited.

  • Gathers and organizes information useful for transition planning.

  • Provides one-to-one assistance to clients in the delivery of instruction for vocational, independent living and personal/social content areas.

  • Supervises and trains students in student-run enterprises.

  • Maintain regular and consistent attendance and punctuality

  • Work collaboratively and communicate effectively with staff and customers at all organizational levels.

  • Perform other duties and responsibilities as assigned
