Job Title: Youth Program & Prevention Coordinator

Hourly Rate: $26.83 - $28.73

General Statement of Duties: Highly responsible position working with the Youth Services Director in developing, implementing and evaluating programs and providing intervention services for youths on a Town-wide basis.

Supervision Received: General supervision and oral direction from Vernon Youth Services Director.

Supervision Exercised: Youth Services Bureau staff in the absence of the Director, supervises various summer programs and staff administered by the department.

Essential Job Functions: Serves as the primary liaison of the Vernon Youth Services.

Assists the Youth Services Director in planning, developing, and implementing prevention programs. Facilitates a variety of youth programs, and other multi-town community-based prevention initiatives. Promotes, monitors and evaluates programs for their success and efficacy.

Provides counseling, crisis intervention, assessment, advocacy, education, support, case management, and referral services for youth. Serves as liaison for families, school support staff to refer youth and families to appropriate community services. Works along-side school staff in identifying and aligning individual programming needs of students.

Builds student relationships and support through positive reinforcement. Assists the Department of Children and Families and the School Resource Officer with cases involving school aged youth. Maintains files and data collection according to State Department of Education and Youth Service Bureau requirements.

Maintains a high degree of confidentiality. Regular and punctual attendance.

Other Job Functions: Attends meetings in absence of Youth Services Director and coordinates program supervision with the Youth Services Director.

Recruits, trains and supervises summer programs and staff. Participates on a variety of school and community committees relative to youth services. Attends pertinent meetings and conferences as needed.

Helps develop collaborative working relationships with other community providers and surrounding towns. Other duties as required.

Knowledge, Skills & Abilities: Knowledge of State of Connecticut Youth Services statutes.

Working knowledge of the relationship between Youth Services and the public education system. Excellent organizational skills. Excellent oral and written communication skills.

Ability to be a self-starter and work independently. Knowledge, skill and ability to work with elementary and middle school aged youth. Ability to work with high school aged youth a plus.

Ability to work in a highly active environment requiring multi-tasking. Ability to work under stressful situations involving family dynamics. Ability to coordinate and foster relationships with school administration, community based professionals, business owners, and the general public.

Ability to develop, run and coordinate programs. Ability to communicate effectively with youth and staff, and develop positive relationships with youth while maintaining appropriate boundaries.

Experience & Education: Bachelor's degree in human services or related field, Master's degree in human services, social work or counseling preferred.

Two to three years experience working with youth programs, preferably in a school setting; any equivalent combination of training, education, and experience. Must possess and maintain a valid motor vehicle operator's license. A Police background check will be required.

VIEW JOBS 4/11/2019 12:00:00 AM 2019-07-10T00:00 POSITION SUMMARY: The Cardiac Rehab Staff Nurse reports to the MMH Cardiac Rehabilitation Site Manager.# The Registered Nurse supervises medically prescribed exercise therapy appropriate to client#s age, medical and emotional/mental status.# The RN provides education classes, concentrating on client#s understanding of the cardiovascular system and heart-healthy life style modifications necessary to maintaining optimum health.# The Cardiac Nurse is accountable for staffing in all areas of patient services under Cardiac Rehab/Cardiac Stress/TEE and is responsible for marketing of program, orientation of the cardiac patient and participating in Quality Improvement. The Cardiac Nurse will participate in the operational duties of the department, such as but not limited to:# Quality Improvement, Policy and Procedure maintenance, revising and development of billing, gathering of statistical data, developing departmental educational opportunities and participating in the on-going growth and maintenance of the excellent customer services provided to all that ECHN serves. # EDUCATION/CERTIFICATION The Registered Nurse must be a graduate of an accredited professional nursing education program Have a current Connecticut license.
