As a Youth and Family

  • Social Worker for our SSBP Program, you will deliver culturally responsive services and lead a collaborative planning process for youth and families who are referred through Sonoma Special Education Plans. As a Facilitator, your primary role is to ensure a sustainable network of unpaid supports (i.e. extended family, mentors, teachers, coaches) are working together toward stability for the youth and family.

ABOUT SSBP PROGRAM

Sonoma's Seneca School Based Partnership (SSBP) Program works with local school districts to provide educationally related mental health services to youth with Individualized Education Plans (IEP). Our staff partner with students' IEP teams to provide a variety of services. Our goal is to address the mental health needs that are impeding a student's progress to academic success. Our services are designed to be short-term and promote long-lasting success. These services, which are child and family focused and strength based, are provided in the office and community, including, but not limited to, schools, hospitals, and family homes

Responsibilities

  • Function as the team clinical leader for respective caseload, and provide direct services that are culturally responsive to youth and caregivers on a weekly basis to further treatment goals

  • Provide psychoeducation, caregiver coaching and coordinate care between providers and school district partners etc.

  • Identify Family Finding and engagement needs and develop Youth and Family teams made up of natural supports for the purpose of treatment planning and sustainable lifelong support.

  • Facilitate Child and Family team meetings (CFTMs) monthly or as needed that maintain compliance to the Wraparound Model.

  • Cultivate and maintain engagement and alignment with school district partners, community members, county partners and other mental health providers.

  • Lead case planning and consultation with team members on clinical issues, as well as casework issues, as appropriate.

  • Complete mental health notes, clinical documentation (i.e. Assessments & treatment plans), safety plans, incident reports, and other documentation accurately and in a timely manner.

  • Participate in the 24 hour on call support line system on a shared rotation.

  • Participate in individual and/or group supervision weekly as well as other required internal staff meetings or trainings.

  • Carries out functions and responsibilities as it pertains to giving orders and conducting evaluations of clients in restraint/seclusion.

Qualifications

Required:

  • Master's degree in Social Work/Psychology/Counseling or equivalent experience.

  • Must be eligible to register with the BBS.

  • Experience working with a diverse population either through professional or personal experience.

  • Flexible schedule with ability to work evenings and weekends.

  • Willing to respond to crisis situations and be part of a rotating emergency pager system.

  • Valid CA driver's license and insurable under Seneca Family of Agencies insurance policy.

  • Clearance of TB test, fingerprints, and any other mandatory State/Federal licensing or certification requirements.

  • Proof of COVID-19 vaccination and booster required.

Preferred:

  • Bilingual English/Spanish.

  • Experience working with youth in out-of-home care.

Schedule

  • Full-time:

  • Mon

  • Fri, 9AM - 5:00PM
  • Flexible evenings and weekends
  • 5 weeks of Paid Time off and 11 Paid Holidays.

Benefits

  • Staring at $69,010- $83,010 per year. Actual salary dependent on creditable experience above the minimum qualifications

  • Salary increases each year

  • Additional compensation provided upon passing bilingual language proficiency exam
