Job Title: Wound, Ostomy, Continence Manager (WOCN)

Job Summary:

Insures the delivery of comprehensive health care in accordance with the philosophy, standards of practice, objectives and policies of the Department of Nursing and the MSHS. Responsible for diagnosis and treatment of common problems related to the treatment, management and rehabilitation of patients with pressure injuries, ostomies, wounds and/or continence problems as well as patient education and competency assessment, counseling and education of nursing staff.

Roles & Responsibilities:

  • Conducts health assessment interview focused on wound, ostomy, continence related care

  • Performs focused physical assessment

  • Develops plan of care based on the diagnosis and individual client needs

  • Recommends medications and treatments as appropriate and works in partnership with medical staff to complete a medical order. Note: A credentialed NP in this role can act independently in writing medical orders

  • Works collaboratively with the health care team and makes referrals to other disciplines as indicated

  • Records, reports and interprets the patient's responses to the plan of care

  • Teaches, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with pressure injuries, ostomies, wounds, or continence problems

  • Interviews and hires Per Diem WOCNs and participates in any termination if necessary

  • Assesses staff nurses knowledge and skills related to wound care management

  • Provides presentations and educational offerings.

  • Demonstrates a professional, courteous and respectful attitude in dealing with diverse staff, patients, including patients' families and significant others


  • Bachelor's Degree in Nursing. Master's Degree in Nursing is highly preferred

  • Graduate of an accredited Nurse Practitioner program , if an NP

  • Graduate of accredited Wound, Ostomy, Continence Educational Program or equivalent

  • Bi-lingual preferred

  • At least two years of nursing experience

  • Nurse Practitioner experience of a least one year preferred

  • Demonstrates leadership abilities

  • Effective organization and interpersonal skills

  • Basic computer skills

  • Ability to problem solve using critical thinking skills

  • Current NY State RN License and Registration

  • Current NY State Nurse Practitioner License and Registration, if NP

  • Certified or eligible for certification by recognized specialty organization

  • Board Certified as a Wound, Ostomy, Continence Specialist or eligible and in process

Take on this impactful role, responsible for proper management of wound care by treating and monitoring wounds that are the result of disease, injury, or medical treatments. The WOCN will provide care that will quickly and safely heal various types of wounds, including surgical openings, ulcers, bed sores, feeding tube sites, and abscesses.
