The Wound Care Nurse is responsible for assessing skin alterations, developing plans of care, implementing orders and treatments, and evaluating progress of specialized skin and wound conditions. The Wound Care Nurse evaluates new admissions, participates in weekly wound team rounds, documents status, insures notification of physician and family of changes, and provides education to nursing staff. The Wound Care Nurse identifies wound types, analyzes trends, which will ensure that patient's needs are met in accordance with professional standards of practice through physician orders, center policies and procedures, and federal, state and local guidelines.

Wound care certification is preferred. Prior wound care experience is required.

Job Requirements

  • Assesses patients by physical examination including pertinent diagnostic testing to determine health status.

  • Administers medications and treatments.

  • Participates in the care planning process and oversees implementation of the plan.

  • Supervises LPNs and nursing assistants.

  • Communicates with physicians regarding changes in conditions, diagnostic test results, etc.

  • Documents assessments and care in compliance with standards of care and company policy.

  • Educates patients and their families on health-related issues.

  • Conduct comprehensive wound consultations including; detailed wound assessments and recommendations for interventions and care planning.

  • Conduct wound assessments for resident and complete all associated documentation, tracking and care planning activities.

  • Ongoing monitoring of all assigned resident with wounds.

  • Assess resident/caregivers knowledge of wound prevention and treatment. Provide resident/ caregiver with wound prevention / treatment education as needed.

ENTRY QUALIFICATIONS

  • Graduate of an accredited school of Nursing.

  • Valid State licensure as an RN.

  • Current CPR certification.

(Full Time) Location: Wilmington The Staff Nurse RN provides licensed nursing care to assigned residents, and supervises non-licensed personnel in the delivery of care to residents. Subordinate staff includes LPNs and CNAs. MAJOR WORK ACTIVITIES: * Asses resident's physical and mental health including reaction to illness and treatment regimens. * Record and report results of resident assessment to physician, other personnel and responsible party as appropriate. * Deliver and record licensed nursing care to include medications and treatments as prescribed by the physician as well as care outlined in the resident's individual plan of care (ADL's, nutrition, hydration, labs retrieval/oversight weight oversight, therapies and restorative initiatives, splinting, assistive devices, skin audits, behavior logs, acute episodic oversight documentation, etc.). * Teach, delegate and supervise non-licensed personnel in implementing the resident's individual plan of care including restorative/functional nursing programs and weekly vital signs. * Ensure continuity of care through documentation and communication of resident status (report) to relieving personnel. * Complete all clinical chart documentation according to best practice at time service rendered, prior to end of current shift, and/or by "late entry" with date, time, and legible signature or initials where appropriate. Documentation that is not time specific/shift specific, i.e. such as MDS, Care Plans, and certain summaries may be completed with signature and date on the day of documentation, completion and/or at time of review. Facilitates and participates in the initial and ongoing care plan and care card updating. * Ensures care plan/care cards are audited to reflect current baseline functionality/initiatives. * Respond promptly to resident needs. * Maintain safe and sanitary work environment. * Participate in Quality of Care and Quality Improvement activities. * Perform specific work duties and responsibilities as assigned by the shift supervisor, Nursing Coordinator, Assistant Director of Nursing and/or the Director of Nursing. Education: Associates Degree in Nursing from an accredited school of nursing Licensure/ Certification: Current North Carolina licensure as Registered Nurse Experience: Some previous geriatric nursing experience preferred.
