Position Responsibilities:

• Identifies patient's needs, Handles unusual, stressful, complex situations with minimal supervision. Promotes optimal patient care.

• Fills in all pertinent columns on every patient on Wound Consult Sheet.

• Conduct all wound care and ostomy consultations within 48 hours of order.

• Demonstrates sound judgment when prioritizing consultations, giving precedence to medically urgent consultations (i.e. severe bed sores, decubitus ulcers, etc.).

• Demonstrates efficient and cost-effective utilization of supplies (dressings, ointments, cleansers, bandages, etc.).

• Works with infectious control and other appropriate departments to develop and implement programs designed to reduce nosocomial skin breakdown.

• Serves as a wound healing/ostomy supply resource, keeping abreast of best practices within the wound and ostomy care discipline and educating patients, family members and other care givers on appropriate wound healing and dressing techniques.

• Provide continuity and help coordinate transition for inpatients requiring outpatient wound Care.

• Supports wound Care operations through commitment to the weekend rotational schedule (required to work every other Sunday in accordance with staffing needs).

• Actively promotes Outpatient Wound Care Department.

• Demonstrates flexibility when needed to float between inpatient and outpatient areas as patient need dictates

• Performs and documents an accurate wound care assessment on consultation or arrival to treatment area.

• Responds to diagnostic data and initiates appropriate action or intervention.

• Assesses patient education needs.

• Assesses psychosocial needs specific to patient's age, growth, and development.

• Formulates plan of wound care to be implemented and monitored collaboratively by wound care nurse and staff nurse.

• Establishes priorities for patient care based on patient needs.

• Verbalizes understanding of patient diagnosis and interdisciplinary plan of care.

• Administers medications; observes effects, recognizes and reports adverse reactions immediately.

• Provides individualized instructions each shift to patient/significant other regarding the plan of care.

• Demonstrates safe and correct use of wound care specific equipment.

• Ensures that each patient's general hygiene needs are met.

• Adheres to standard precautions and hand hygiene guidelines at all times.

• Performs dressing changes using aseptic technique.

• Collects and labels specimens per policy when ordered by physician.

• Performs bedside glucose monitoring according to policy.

• Reports all abnormal findings immediately to physician.

• Initiates wound and skin protocol upon identification of actual or potential skin breakdown.

• Performs ostomy care to include appliance usage, and reinforcement of teaching.

• Inserts intermittent or indwelling urinary catheter using aseptic technique.

• Documents/reports patient responses or outcomes resulting from: nursing interventions, treatments/procedures, interpersonal interactions, and patient/family education.

• Evaluates and documents patient's progress towards established goals/objectives.

• Updates/revises patient care problem list and individualized patient care standards based on patient/family responses, progress towards goals, and/or change in medical status on each inpatient assessment.

• Evaluates patient length of stay and related transfer/discharge planning needs based on anticipated problems/needs, internal or community resources, and support systems.

• Plans, presents, and documents unit in-service as required by level.

• All other duties as assigned.

Qualifications:

• FL RN License

• Required

• BLS and ACLS

• Required

• Wound care Certification preferred
