Under the direct supervision of the Wound Care Registered Nurse or the Chief Nursing Officer designee, provides patient with care that is continuously evaluated, planned for, and revised as needed. The LVN/LPN Wound Care Nurse conducts all required wound care activities according to hospital policy and national standards. Collects the data from a comprehensive wound assessments and carries out the plan of care including goals, implementation, evaluation of outcomes for patients ranging in age from 18 years to geriatric. Provides patient, family and staff education regarding prevention of pressure ulcers and therapeutic support services, dressing selection and adjunctive therapy. Collaborates with clinical staff, physicians and other members of the treatment team to provide the patient with the highest quality and most appropriate wound care. Acts as a resource for the treatment team and clinical staff and assists with staffing as assigned. Trains and manages the Wound Care Team Members to include orientation, and in-service training. Coordinates with the interdisciplinary Wound Care Committee as applicable.

Education and/or Experience:
Graduate from an accredited school of licensed vocational nursing or licensed practical nursing with current state license in good standing.
Two or more years of acute care or rehabilitation.
Two or more years of wound care experience as a LVN/LPN is preferred, including collection of wound data, recommendation of treatment plan, recommendation of appropriate wound dressing, documentation, , education, data collection, surveillance and outcome data.
Knowledge of principals of infection control.
