Schedule is Mon-Fri 6a-2:30p
Provide a continuum of comprehensive, coordinated interventions to pregnant and post-partum women impacted by substance use
Conduct substance abuse/mental health co-occurring screenings (in-home or agency)
Provide intensive case management, supportive counseling, and community referrals
Part of a multi-disciplinary team that is engaged in integrated healthcare activities
Position includes transporting clients to social service and medical appointments, as needed, in an agency vehicle
Valid Florida driver's license required
Must be 21 years of age or older
Must pass a DOT physical exam and have the ability to operate a vehicle
