VAMC Tomah (http://www.tomah.va.gov/) has been improving the health of the men and women who have so proudly served our nation. We consider it our privilege to serve your health care needs in any way we can. Services are available to veterans living in a Western/Central area of Wisconsin.
Ninety Day orientation at Tomah VAMC. After orientation, time will be split between Tomah VAMC and Wisconsin Rapids Community Based Outpatient Clinic (CBOC) until new outpatient clinic is built in Wisconsin Rapids.
Wisconsin Rapids clinic is complete the position will have a duty location of Wisconsin. Rapids CBOC. Wisconsin Rapids is considered to be within the local travel radius of 50 miles.
Travel within the local travel radius is not reimbursable. Incumbent must report at designated site upon start of tour of duty daily as directed and travel time is not part of the scheduled tour of duty.

This developmental position, located in the Patient Care Service, Whole Health Program, at the Tomah VA Medical Center, Tomah, WI. The position composes a career ladder structure to the full performance level of Social Science Program Specialist, GS-0101-09.
This positon is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing, and Whole Health clinical care elements of the Whole Health Systems. The position provides care to Veterans seeking self-directed, lasting changes aligned with their values. They provide interventions that promote health and wellness and enhance well-being, improve health related outcomes, reduce likelihood of inpatient admission, and improve quality of life.
Provides health coaching for Veterans
Contacts eligible participants for enrollment and explains the health coaching process
Conducts interventions for Veterans
Conducts Whole Health Facilitated Groups; MOVE, Smoking Cessation, Diabetes Management, Hypertension Clinics, Mental Health, and other inpatient and outpatient groups as appropriate
Provides follow-up care through telephone clinics, secure messaging, or face-to-face appointments
Actively guides Veterans in the exploration of core elements of personal health planning through individual or group based programs
Reviews current health behaviors and develops customized treatment plans with Veterans
Utilizes motivational interviewing and health coaching techniques and processes
You may qualify based on your experience and/or education as described below:
Degree in behavioral or social science; or related disciplines appropriate to the position.
Combination of education and experience -- that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.
Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.
NOTE: In addition to meeting the above qualification requirements, applicants must have specialized experience and/or directly related education in the following amounts.
GS-7: You must possess at least one year of specialized experience at the GS 5 level, that has equipped you with the knowledge, skills and abilities to perform successfully the duties that is typically in or related to the work of the position to be filled.
Examples of specialized experience include but are not limited to demonstrated experience in: Skill in health coaching techniques, motivational interviewing and health coaching techniques and process; skill in obtaining specific factual data or opinions on specific issues; Skill to managing weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, and pain management.
Education: You must possess one (1) year of graduate-level education or superior academic achievement.
Do you possess an equivalent combination of education and experience? (TRANSCRIPTS REQUIRED)
GS-9: You must possess at least one year of specialized experience at the GS 7 level, that has equipped you with the knowledge, skills and abilities to perform successfully the duties that is typically in or related to the work of the position to be filled.
Examples of specialized experience include but are not limited to: Skill in developing intervention strategies to help clients; Schedule ongoing sessions to assess progress; Skill in coordinating care with treatment providers; Skill in motivational interviewing and health coaching techniques a process. Maintains education and knowledge regarding lifestyle and clinical outcomes, including behaviors that improve patient's well-being and those related to poor clinical outcomes; Skill in developing and conducting onsite educational series for enrolled patients.
Education: You must possess two (2) years of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree.

Part-time experience will be credited on the basis of time actually spent in appropriate activities. Regular and recurring work is mostly sedentary involving sitting at a desk, although the positon requires the ability to move from one geographic location of the facility to another.
