West Virginia - Pmhnp - Adult And Geriatric Units

Forefront Telecare is looking for PMHNPs that are licensed and has a DEA in West Virginia to work remotely in a Psychiatric hospital.

Full-time and part-time working weekdays and weekends.

You will be covering weekday and weekend rounding, call, providing emergency medication call, and initial assessments over the weekend.

30 Bed Geriatric Unit- 20 Bed Adult Unit- 10 Bed Adult Psych ICU

What you will do/Your responsibilities:

• Conduct psychiatric assessments/evaluations and diagnose clients according to DSM-5 or ICD-10 criteria
• Formulate a working diagnosis, develop, and implement at treatment plan, and evaluate/modify therapeutic regimens to promote positive patient outcomes
• When clinically indicated, perform assessment scales (e.g., PHQ-9, GAD-7, CSSR-S, etc.), order laboratory tests, review results, and document as required by Forefront Telecare guidelines
• Prescribe, titrate, and monitor psychotropic medications
• Participate in a collaborative approach of patient management using the knowledge and clinical resources of every member of the health care staff to provide the resolution of diagnostic and therapeutic problems and the implementation of effective solutions
• Consult with members of the clinical team as appropriate
• Provide individual and/or family education to increase each patient's awareness of illness and treatment issues
• Participate in administrative or clinical meetings during scheduled hours
• Perform activities and discussions related to coordination of care (PCP communication, calls with primary care practitioner, referral to higher or lower level of care etc.)
• Organize care delivery in collaboration with the interdisciplinary team in a manner that promotes continuity and accountability for patient care
• Promote team approach to patient care through creating and maintaining professional working relationships with physicians and support staff
• Maintain appropriate medical records and complete all necessary clinical documentation in an accurate and timely manner in accordance with Forefront Telecare guidelines
• Video(Zoom) conference with patients who have psych needs from your remote, secure location.
• Evaluate and complete notes via EMR system in a timely manner.
• Proficient in patient interview and mental status exams
• Collection of comprehensive histories including diagnostic impressions
• Pharmaceutical management
• Safety evaluations

Requirements

• At least 2-3 years in psychiatric adult and geri unit required
• Have an active license for the states which you are applying
• Board Certified (PMHNP-BC) by ANCC
• Clean NPDB
• Multi-state licensed preferred
• TelePsych experience preferred
• Updated CAQH preferred
• Must be able to provide 2 years of HIPPA compliant case/activity logs
• Active DEA Controlled Substance Registration in West Virginia
• Friendly and compassionate disposition
• Effective communication skills
• Team player
• Knowledge of evidenced based interventions are required to care for patients of all ages who are experiencing different behavioral health crises and substance abuse.
