Job Summary

Involves participation and implementation of program goals consistent with preventative and holistic care, having objectives to promote a strengths-based, wellness-informed client-centered approach to assisting individuals in achieving person-driven and self-management goals.

Roles & Responsibilities:

  • Provides individual and/or group training in healthful living, self-management of chronic mental illness, well living skills, creation of healthful lifestyles, and support in areas such as self-care, holistic wellness, hygiene, and other life skills as needed.

  • Provides linkage and liaison with community resources as needed.

  • Meets with clients individually and/or group basis to establish realistic treatment goals, prioritizing the goals and reviewing goal attainment.

  • Documents in clear and concise fashion to reflect assessment, treatment planning and implementation information that is measurable and functional to reflect what the client's expected outcome will be.

  • Demonstrate the knowledge and skills necessary to provide care, based on physical, psychosocial, education, safety and related criteria, appropriate to the age of the client.

  • Provide supportive counseling to facilitate problem solving.

  • Collaborates with team members to develop and implement treatment plans involving multiple modalities.

  • Participates in team rounds, weekly conferences, supervisions and other pertinent meeting with interdisciplinary team to review client's progress and plan and review treatment.

  • Document medical record(s) in a timely and accurate manner and meets the regulatory agencies documentation requirements.

Education Requirements:

A Graduate Degree in social work or other related disciplines

Experience Requirements:

Prior working experience with individuals with severe and persistent mental illness

Computer Skills:

Microsoft Office - Intermediate
