Position Mission: The Wellness Nurse is a licensed nurse responsible for coordinating resident care and medication management under the medical direction and supervision of the residents attending physicians in order to ensure the resident remains as independent as possible for as long as possible. The Wellness Nurse works on the scheduled days off of the Health Services Director to provide the community with consistent coverage from a licensed medical professional seven (7) days a week.

Responsibilities:

  • Communicate with community healthcare professionals, including physicians, discharge planners, nurses and state agencies on the care needs of all residents at the community.

  • Coordinate incidental medical, dental, vision, hearing and podiatry care for residents.

  • Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by a physician.

  • Provide support to Medication Technicians to ensure competent delivery of medications to residents.

  • Assist with resident assessment tasks as directed by the Health Services Director.

  • Conduct staff interviews and observe residents on a continual basis to ensure community staff is informed of changes in residents physical and emotional health. Communicate all relevant resident information to physicians and other healthcare professionals. Alert Executive Director and responsible party to concerns as needed.

  • Complete thorough documentation in the resident chart on an ongoing basis according to company policy and state regulations.

  • Review and monitor staff use of the Shift Report daily and participate in the change of shift meeting to communicate pertinent information regarding resident care needs as needed.

  • Coordinate prescription orders with doctors offices and pharmacy delivery of medication. Oversee the central storage and delivery of medications.

  • Communicate with residents and responsible parties regarding the communitys preferred pharmacy and incontinence supply programs.

Qualifications:

  • Must be a Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing

  • Prefer one (1) year experience working in Residential Care Facility for the Elderly (RCFE)

  • Able to safely operate standard household equipment (dishwasher, iron, vacuum, washer, dryer)

  • Able to operate a personal computer and be proficient in MS Office programs (Word, Excel, etc.)

  • Able to operate standard office equipment, including fax machine, copier, telephone, etc.

  • Knowledge of and/or ability to learn the theory and practice of assisted living and dementia care

  • Able to work with seniors and patiently interact with cognitively impaired individuals

  • Able to obtain and maintain valid first aid and CPR certification

  • Able to obtain and maintain a valid Food Handlers certification, as needed

  • Be mobile and able to perform the physical requirements of the job. Good physical health, verified by a health screening, including a chest x-ray or an intradermal test result not more than 6 months prior or seven (7) days after employment

The Medication Nurse is responsible for administering injections, suppositories, enemas, and medications associated with wound care in addition to medications usually assigned to a Medication Technician. * Perform nursing duties associated with the Wellness Nurse role as assigned. * Coordinate medication services with resident, pharmacy and physician. * Order and track all medication changes, refills and deliveries with pharmacies. * Prepare medication for distribution to residents as per company policy. * Assist residents with taking their prescribed medications. * Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal and notify supervisor. * Maintain accurate, complete and confidential resident medication and care records according to company policy. Ensure all medication documentation is current and correct, including medication administration forms, physicians orders and dosage changes. * Keep medicine rooms and carts orderly, clean and appropriately stocked. Maintain an inventory of all department supplies and re-order as needed. * Control medication room/cart access at all times. * Monitor Psychotropic medication use is congruent with physicians orders and ensure resident behaviors warrant the use of medication. * Participate in and complete narcotic count at beginning and end of each shift. Report any and all discrepancies to supervisor immediately. * Communicate with physicians, family members and vendors, as needed. * Understand the meaning and usage of common medical abbreviations, symbols and terms relative to the administration of medications as used by the company, pharmacists and physicians.
