Job Description: Wellness Coordinator- RN, LPN or Registered Dietician (Part-time up to 24 hours weekly)

Job Summary

Innovative Wellness is a rapidly growing practice with the mission of promoting healthy lifestyles that in turn support a work environment that is conducive to success. Our Wellness Coordinator helps to build, plan for, coordinate, and deliver health promotion activities designed specifically for our client's company culture and member population. Whether it's a company-wide or individual solution, our programs develop a wellness relationship with our members that fosters positive behavior change and customer satisfaction. The goal of Innovative Wellness is to improve the health and wellness opportunities and outcomes of the member populations we serve while increasing accessibility of care and reducing costs.

Essential Job Responsibilities:

Health Coaching

  • Assess patients' and clients' nutritional and health needs.

  • Counsel patients on nutrition issues and healthy eating habits.

  • Develop meal and nutrition plans, taking both clients' preferences and budgets into account.

  • Helps provide individual wellness solutions to our members in an effort to cultivate long-lasting, healthy, lifestyles.

  • Uses Account and Project Management skills to guide members and their expectations.

  • Assists in providing analysis of biometric results, pharmaceutical usage, health risk assessment data and other disease analytics and deliver targeted and general health education to members.

  • Engages members with weight management, tobacco cessation, stress management, nutrition, sleep, physical activity, managing chronic conditions, and other healthy lifestyle behavior change programs.

  • Coaches members to make long-lasting behavior changes to achieve and maintain optimal health and well-being.

  • Identify co-morbid conditions and other factors that could impact the support needs of the members, outside the scope of the health coaching program.

  • Conducts company culture audits to identify programs and events that fit within the culture and values of our wellness members.

Wellness Coordinating

  • Assists in Designing population health programming and coordinates and delivers comprehensive population health programming to foster behavior change and maximize engagement, outcomes and customer satisfaction with Innovatives's wellness initiatives.

  • Assists with developing client "Strategic Wellness Calendars" that schedule targeted activities and events for the upcoming year.

  • Schedules, organizes and oversees health screening, fitness or wellness events, challenges or informational programs, to ensure their success.

  • Schedules biometric testing.

  • Communicates and partners with key vendors effectively, to ensure key deliverables.

  • Demonstrates, initial setup, and on-going processes related to Innovative's wellness portal.

  • In conjunction with Innovative Marketing, helps to design communication and marketing materials to promote client health initiatives and wellness programs.

  • Keeps abreast of current industry, market, and policy information.

  • Recommends or approves new programs or service offerings to promote wellness and fitness.

  • Works with compliance council to ensure wellness programs and incentives are compliant with national standards and regulations.

Skills & Competencies

  • Is a Registered Nurse, Licensed Practical Nurse or Registered Dietician

  • Has a sense of urgency and ownership.

  • Is a self-starter.

  • Has a working knowledge of HIPPA regulations.

  • Demonstrates effective Project and Account Management skills.

  • Background and/or experience with healthcare industry, medical claims

  • Knowledge of specific and lifestyle-related topics such as smoking cessation, weight management, nutrition, stress reduction, physical activity and chronic conditions.

  • Experience working with individuals to facilitate effective behavior change in the areas of physical activity, tobacco cessation, nutrition, stress management, sleep and chronic condition management.

  • Is proficient with the following programs: Word, Excel, Outlook, and PowerPoint.

  • Is proficient with Salesforce.

  • Is flexible with working hours and when necessary, works any hours necessary to meet a deadline.

  • Demonstrates a desire to advance within the organization.

  • Implements and follows up with work assignments.

  • Effectively operates independently.

  • Effectively operates in a fast-paced and dynamic environment.

  • Demonstrates exceptional client-facing skills.

  • Demonstrates excellent presentation skills and the ability to relate to our client's, leadership groups.

  • Possesses the means to travel locally to onsite locations for consultations, presentations, or health fairs.

This position is part time offering 16-24 hours weekly with the potential to transition to full time.

Essential Duties and Responsibilities: Maintains and manages all supplies, including ordering, purchasing and stocking medical supplies. Accurately report non-formulary supplies and quantity to Executive Director (ED), and maintain sufficient quantity to ensure availability. Provide timely and accurate tracking of supplies and utilization. Dispense supplies as requested by DON and Charge Nurse. Routinely rotates all supplies per FIFO and ensures supplies are securely stored.
