Overview
HSL provides enhanced housing with services at affordable housing locations--some of which are our own and others of which are partner sites. For this role, you will work with residents at a Boston Housing Authority building called Martin Luther King (MLK) Towers in Roxbury, MA. You will partner with Boston Housing Authority staff, Central Boston Elder Services staff, and community partners to provide a robust program of supports. At its core, enhanced housing with services is a proactive approach wherein resident services staff regularly and actively reach out to each individual resident to engage with them around their health and wellness, identify areas of need/risk and provide intensive, individualized support as needed and desired by the resident.
The Wellness Coordinator works in partnership with housing staff to engage residents in wellness assessments and health education programs, connect residents to needed services, provide a wide variety of case management tasks, ensure that all interventions are documented and tracked and to develop and strengthen partnerships with community provider organizations as needed. The Wellness Coordinator is part of the housing team working to support the residents in living independently and safely for as long as possible. The Wellness Coordinator at MLK Towers serves as the face of Hebrew SeniorLife on this campus, forming partnerships with outside partners and internally with the housing team.

Required Qualifications
Must have compassion for and a desire to work with a senior population.
Masters degree in a human service field and 1-2 years of experience working as a case manager or care coordinator in aging services preferred; Bachelors degree required with some experience working with seniors
Excellent organization and interpersonal skills. Must have ability to innovate, think strategically and conceptually, manage multiple projects simultaneously and handle difficult situations.
Must be a professional, proactive, collaborative, conscientious, and results-oriented individual.
Must have an optimistic and positive demeanor, excellent oral and written communication skills, good intuition and sound judgment.
Must be motivated to learn and flexible to change.
Computer literacy; experience with Windows, Word, Power Point, and Excel.
Preferred Qualifications
MSW or experience in health coaching or case management preferred
Certification in Dementia Care preferred.
