The incumbent will underfill the Social Worker 2 position and will advance to the next level in the series upon recommendation of the Appointing Authority, meeting the minimum qualifications and satisfactory performance. This position will be primarily responsible for providing case management for recipients in the Home and Community Based Waiver (HCBW) program for the Frail Elderly (FE) and Persons with Physical Disabilities (PD). This includes interviewing and assessing individuals to determine if the criteria are met for placement on the HCBW and providing case management through other Medicaid waivers, and State Funded programs such as Personal Assistance Services (PAS), Community Options Program for the Elderly (COPE) programs and Homemaker Program.
This position will complete monthly contacts with program recipients, complete required reports to appropriate agencies, complete documentation in set databases and complete community program referrals as needed. Education and Experience (Minimum Qualifications) Licensure or provisional licensure as a Social Worker, Clinical Social Worker, Independent Social Worker or Associate in Social Work. Special Requirements A valid driver's license is required at the time of appointment and as a condition of continuing employment.
A pre-employment criminal history check and fingerprinting are required. Persons offered employment in this position will be required to pay for fingerprinting. Licensure or provisional licensure by the State of Nevada Board of Examiners for Social Workers as a Social Worker, Clinical Social Worker, Independent Social Worker, or Associate in Social Work is required at the time of appointment and as a condition of employment.
