  • Responsible for relief primary patient care for patients in all geographic areas of the agency's service area. Must be available when the office is closed during weekday and weekends. Weekend RN will report to work on Friday at 10:00 am. Weekend RN responsible for responding to all beeps, after hours, and PRN visits from 5pm Friday (8am if office closed on Friday) until 8:00 am Monday. For weekday RN, from 5pm Monday (8am-8am if office closed) until 8am Friday. Responsible for administrative call duty and receiving new referrals from discharge planners, entering new referrals into Delta, completing telephone visits, and providing care if needed.

  • Collaborates with all caregiver staff: team RNs, PT, OT, ST, MSW for interdisciplinary coordination of patient care; provides field supervision of HHAs; works interdependently with Clinical Coordinators, Scheduling Coordinator, Data Entry and Billing Specialists and clerical support.

  • Directly accountable to the Administrator/Director of Clinical Services. Functions within agency and unit policies and procedures. Manages own time productively and independently and complies with attendance and reporting requirements.

  • Knowledge of State Licensure Laws, the Conditions of Participation of the Federal Health Insurance Program for the Aged and Disabled (Title XVIII, Social Security Act, Section 1861), and the regulations relating to Conditions of Participation for Home Health Agencies (Part 405, Section 1201-1243), JCAHO and OSHA regulation desirable. Must have foundational knowledge of medical-surgical nursing practice.

  • Demonstrate ability in communication and interpersonal relationships. Good writing skills and computer literacy are desirable. Organizational and time management skills desirable. Must have strong physical assessment skills and clinical skills.

  • Must be highly adaptable and have the ability to exercise a high degree of initiative and judgement in carrying out work assignments. Is self-directed and seeks and acquires the information needed to function efficiently and effectively within a clinical model that incorporates the concept of coordinated patient care. Must have good critical thinking and problem solving skills.

  • In the office minimal. Must use own car when company transportation is not available for travel to remote sites, often in rural, isolated areas. Must be able to handle potential problems related to safety issues for staff and patients in all settings.

  • Must be familiar with the principles of growth and development over the life span and the ability to assess data reflective of the patient's status and interpretation of information needed for each patient's requirements relative to his/her age specific needs to provide the care needed.

  • Must have responsible, ethical, self-initiating professional behavior. Effective interpersonal skills, empathy and ability to listen and plan with staff, patients/customers/physicians are essential characteristics. Contributes to a positive work climate and to the overall team effort in a manner, which exemplifies the values and philosophy of the organization. Must be able to efficiently communicate with a diverse population. Self-awareness and self-evaluation are important characteristics as are accuracy in collecting data, completion of records and follow-up.

  • Adheres to dress code. Presents a neat, clean, well-groomed appearance.

  • Adheres to department attendance policy. Demonstrates punctuality and good attendance.

Required Skills


  • At least two years of nursing experience preferred.

  • One year home care highly desirable


  • Must possess a current valid SC driver's license and automobile insurance as required by law. Must be at least 21 years old to be covered under McLeod Health's fleet vehicle insurance policy. Must meet McLeod policy for driving eligibility. License and registration as required by state law. RN License, BLS Certification. Possession of professional liability insurance is encouraged. Graduate of an accredited program in nursing

Additional Details * Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapyplan of care. * Provides services within the scope of practice as defined by the state laws governing the practice of physical therapy, inaccordance with the plan of care, and in coordination with other members of the health care team. * Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/orthe physician. * Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptablestandards of practice. * Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the planof care, functional limitations, and progress toward goals. * Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes per LHCpolicy. * Completes all patient evaluations and develops the PT plan of care within state specific guidelines. Reports outcomes ofevaluation, goals, and anticipated projected frequency of care to supervising nurse within 24 hours of the evaluation. * Participates in staff conferences and committees as necessary. * May supervise home health aides in lieu of a RN if physical therapy is the only skilled service being provided and if allowed perstate regulation. * Provides supervision of the PTA as per LHC policy. * Prepares a written discharge summary and/or a written summary report on patient's condition at least every 60 days. * Provides in-service education as requested by agency leadership. * Adheres to LHC Therapy Standards of Practice in provision of services to all patients. * All other duties as assigned.
