The Center for Medicare and Medicaid Services (CMS) recognizes Chronic Care Management (CCM) as a critical component of patient care that contributes to better health and care for individuals. Under the program, patients with two or more chronic conditions receive remote care management for medication reconciliation, prescription refill help, care coordination, etc. The services are covered under Medicare as long as the patient meets the criteria of the program. iSalus' CCM offering provides the clinical services to administer a CCM program for its practice clients, thus helping physicians not only take better care of his/her patients, but also providing revenue growth for the practice through an added service line.
iSalus realizes we have a significant opportunity for growth in our CCM business with focused leadership in the current business offering, as well as the possible introduction of other congruent solutions. We are looking to add to our executive team that individual who can lead and grow our CCM business to new heights through focused leadership.
iSalus headquarters are located in Downtown Indianapolis, IN., but this role can be based anywhere, with a preference for the Eastern or Central time zones. We will be working remotely through at least December 2020, but as the office reopens, this role will require some in-office work in Indianapolis.
In conjunction with the President, develop, manage, monitor, and improve iSalus's clinical CCM program
Assess current business and clinical processes and suggest/implement positive change leading to increased patient conversions into the CCM offering from our client practices
Build a team that will add Net New practices that will grow the CCM lives covered
Ensure the CCM program is delivering on its promise to improve patient care for our physician practice clients
Engage with existing and new clients to continuously improve the CCM onboarding and practice retention strategies and processes
Serve as the cultural leader for the CCM organization, inspiring a strong collaborative and mission-driven approach to care with the aim of transforming the lives of patients
Partner with operational leaders to define success metrics, identify key opportunities for clinical value and growth, and tailor care model components according to policy and procedures
Collaborate cross-functionally with senior leaders to set the strategy and be a true thought partner in carrying out iSalus's mission.
Experience and Skills:
Has deep knowledge of remote Chronic Care Management and Utilization Management practices.
Has established operational processes to meet the demands of growth in a rapidly scaling clinical care offering
Has grown a business from $2M to $20M+
Has strong business acumen to take an established, profitable business line and build infrastructure and process to aggressively grow and scale the business.
Has demonstrated success with operational, commercial, and P&L experience running an outsource/service business with a clinical focus.
Possesses a can-do mentality; excited about the opportunity to "roll their sleeves up" and change the healthcare system
Demonstrated track record of building and mentoring highly collaborative care teams; can attract, develop, and retain A-level talent
Ability to connect and build meaningful relationships with clients
