WHAT IS THE VOLUNTARY RELOCATION PROGRAM (VRP)? The Voluntary Relocation Program (VRP) is a negotiated process which affords current Bargaining Unit employees the opportunity to relocate to a Post of Duty (POD) to a position that is the same as their home position of record, same grade, series and PD. Relocation must be an exact position match, the program does not allow for promotion or change to lower grades.

Applicants will ONLY be referred for locations which they wish to be considered. Applicants can request a copy of the position description if selected under this program.

VIEW JOBS 1/12/2019 12:00:00 AM 2019-04-12T00:00 GENERAL SUMMARY Functions as a member of FACT, a self-contained Clinical team, which provides needed treatment, rehabilitation and support services to identified "members" with severe persistent mental illness. The Principles of Recovery, empowerment and person first language will be incorporated into all activities that support the PACT model. Establishes, administers, and directs the Program of Assertive Community Treatment (FACT), a self-contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified members with severe and persistent mental illnesses; supervises and evaluates the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful and respectful services to program members; and functions as a practicing clinician on the team. MINIMUM QUALIFICATIONS The team leader has at least a Master's Degree in social work, psychiatric rehabilitation, or psychology, or is a psychiatrist; and is licensed in Florida in one of these professions. Also, may be a Registered Nurse. Has strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses. Supervisory and program management experience is desirable. The person in this position must have a clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses and be knowledgeable about the FACT model. Service provision is to comprise between 25-50% of Team Leader's time. PHYSICAL REQUIREMENTS * Must be in good physical condition with no restrictions of movement. * Must be able to apply Therapeutic Effective Aggression Management (TEAM) Techniques without limitations. ESSENTIAL JOB FUNCTIONS 1.0 Direct the day-to-day clinical operations of the FACT team including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours; lead the daily organizational staff meetings and treatment planning meetings. 2.0 Collaborates with FACT psychiatrist. 3.0 Practices in fidelity to PACT standards as set forth in the PACT Model for the Community Based Treatment of Persons With Severe and Persistent Mental Illness, except in such cases that conflict with applicable laws or specific contract language. 4.0 Direct and coordinate, for each member, the admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Schedule the admission interview; develop and coordinate the initial assessment and initial treatment plan; assign the most appropriate staff to the individual treatment team (ITT) and provide clinical supervision of the development of the comprehensive assessment and the treatment plan for each member. 5.0 Continuously evaluate the status of members and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs. Assure that formal recovery plans and daily organizational notations are updated to insure that services are provided according to the most recent assessment and plan. 6.0 Set up and maintain appropriate FACT headquarters with suitable furniture, office equipment, and telephone lines, directly or by delegation. 7.0 Develop and administer the FACT program budget including line items to purchase necessary furniture, equipment, communications devices (e.g., telephones, cellular phones, beepers), and supplies and to cover travel and transportation (e.g., purchase or lease program cars, or cover mileage and insurance for use of personal staff vehicles or both). 8.0 Supervise medical records management assuring maintenance of the medical record in compliance with agency policies, Medicaid, and other third party payment requirements, train staff on medical record requirements; regularly review member assessments, treatment plans, and progress notes written by the staff; and supervise individual staff for medical records mastery. 9.0 Carry out and document quality-assurance activities and reviews of use of program services. 10.0 Develop and maintain program policies and procedures and revise as necessary. 11.0 Initiate and maintain relationships, in coordination with other staff, with law-enforcement and other human service agencies, and with informal community resources (e.g., landlords and employers). 12.0 Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to members. 13.0 Perform on-call duty for crisis intervention and for staff supervision and consultation. 14.0 Performs other duties as requested. 14.1 Miscellaneous duties are performed according to instruction.
