Under the direction of the Technical Supervisor, Laboratory Director, and the administrative direction of the Director of Laboratory Services, and within established organizational and departmental policies and procedures, responsible for supervising and directing the Hematology Section and the Point of Care Testing program.. Also assist in problem research and resolution for customer service (physician, medical office and hospital staff primarily) problems in the functional areas of Hematology and point of care testing.
The VNA & Hospice Intake RN functions primarily in the office taking referrals for the agency, coordinating admissions, and maintaining relationships with referral sources and serves as the front line representative of Cooley Dickinson VNA & Hospice. The Intake RN informs health care providers, patients, and families and the community at large of the benefits of services available through home care/hospice programs and seeks out opportunities to increase referrals to the programs of the Cooley Dickinson VNA & Hospice. The Intake RN receives, evaluates, and coordinates referral activity and distributes information to appropriate team for scheduling of assessment/admission visits.
The Intake RN interacts primarily with social workers, case managers, physicians, and other referral sources and discusses Cooley Dickinson VNA & Hospice programs and services with adults and geriatric patients and their caretakers, including making visits to facilities as appropriate. The Intake RN identifies problems and adverse referral situations and works collaboratively with referral sources and agency staff to correct same while partaking in case conferencing on an inter-disciplinary level and internal to the department.
This position reports to the Director, Business Development and operates within established organizational and departmental policies and procedures.
MINIMUM REQUIREMENTS:
Graduate of an accredited school of professional nursing required
Current licensure as a Registered Nurse in the Commonwealth of Massachusetts required
Minimum of one (1) year recent nursing experience with at least one (1) year in home care/hospice preferred; two (2) to four (4) years medical/surgical RN experience preferred; home care/hospice and supervisory experience strongly preferred
Demonstrated knowledge and understanding of the dying process with an ability to identify and treat symptoms required.
Excellent communication skills, interpersonal skills, and documentation skills required, including the ability to interact with current and new referral sources to promote agency referrals required
Demonstrated organizational and time management skills, critical thinking, and an ability to work independently in a fast-paced environment required
Demonstrated proficiency in computer data entry skills required
Current driver's license, a good driving record, and reliable transportation required
