Job Summary

Conducts patient interviews and abstracts clinical data from the patient's medical record to complete and submit State mandated birth and death records, electronically or via mail, to the appropriate agency, maintaining confidentiality of all data and related information. Maintains records/logs of birth, fetal death/stillbirth, abortion registry as well as deaths.

Minimum Job Qualifications

  • Licensure or other certifications: Must obtain Notary and Vital Record Registry within three (3) months of hire.

  • Educational Requirements: High School Diploma or GED.

  • Minimum Experience: Two (2) years experience in a related healthcare setting or related role.

  • Preferred Licensure or other certifications: Deputized as a State Vital Record Registrar; Public Notary for paternity acknowledgments and other Vital Record documents.

Job Specific and Unique Knowledge, Skills and Abilities

  • Knowledge of vital records requirements mandated by State regulations

  • Must have strong knowledge of medical terminology and anatomy and physiology to abstract required clinical information to complete vital record documents

  • Data entry/word processing skills for completion of typed and on-line vital records forms. Types 50 wpm.

  • Strong communication skills for communicating with staff, patients and families, obtaining vital record information and completion of all required documents. Interviews conducted in person and/or via telephone.

  • Bilingual - preferred

  • Must be self-motivated and have good communication (verbal and written) and customer service skills

  • Requires critical thinking, analytical, work prioritization and organizational skills

  • Legible and neat handwriting required; Vital Record Logs are handwritten and permanent legal documents

Essential Tasks and Responsibilities

  • Conducts patient interviews and assists patients with completing State mandated vital records for all hospital births, fetal deaths, infant deaths, stillbirths and abortions.

  • Abstracts clinical data (prenatal and pregnancy history, medical complications, labor and delivery complications, congenital malformations) to complete vital records for all hospital births, fetal deaths, infant deaths, stillbirths and abortions. Enters all pertinent information into GAVERS.

  • Recognizes inconsistencies in documentation by the Physician and/or patient care services.

  • Maintains electronic and manual register of births, fetal deaths, stillbirths and abortions, birth defects registry and computerized hospital death log, routinely submitting to appropriate agencies.

  • Assures delivery logs are completed and maternal and/or infant patients are registered appropriately.

  • Communicates with appropriate departments to ensure corrections and deletions are made.

  • Releases information to appropriate agencies (i.e. Hall County Health Department, Lifelink Organ Procurement, coroner's office) in a timely manner.

  • Prepares, proofs, and distributes birth, infant death, fetal death/stillborn abortion certificates.

  • Reviews all vital record requests for invalid or inappropriate requests for information; communicates and manages situation appropriately.

  • Determines what documents are required by the state of Georgia based on patient's marital status.

  • Notarizes vital record documents for completion of paternity acknowledgments and Confirmation of Birth.

  • Searches and locates required medical records for filing of vital records worksheets, updating death or birth records and completion of research and special projects utilizing Horizon Patient Folder EMR System, EPIC, and knowledge of HIM department medical record processing and flow procedures.
