Occupational Summary

Perform a variety of hemodynamic evaluations to detect obstructions, as directed by a physician or in response to previous test results, including: carotid duplex ultrasound imaging, arterial duplex imaging, venous duplex imaging, and doppler ultrasound, plethysmography, pressure measurements, and other methodologies to evaluate patients suspected of having peripheral vascular disease. Provide excellent service to our patients along with a written preliminary interpretation and impression of test results that are complete and accurate. Record and maintain test data.

Work Performed

  • Assess patients condition/information and determine the appropriateness of procedures to be performed as required by the Intersociety Commission for the Accreditation of Vascular Laboratories and to establish medical necessity as mandated by HCFA.

  • Evaluate patients for the presence or absence, severity and location of peripheral vascular disease by selecting the most effective procedure(s) from an array of anatomic, hemodynamic and physiologic procedures including duplex scanning, plethysmography, doppler ultrasound, pressure measurements and active or passive hyperemic challenge. Record test results and report the results to the physician upon completion of testing.

  • Document and prepare patient records for review and interpretation by physician, including the selection, recording, cross-referencing of significant or representative studies.

  • Prepare a complete and accurate written report for subsequent review and signature of the medical director or his/her designee.

  • Maintain confidentiality of patients, employees, and related activities.

  • Participate in quality improvement activities: evaluate procedure results for technical quality and accuracy; maintain appropriate logs, files, and records.

  • Maintain equipment to assure conformance with specific operational standards.

  • Maintain equipment in working order, making minor adjustments and repairs, perform routine cleaning and preventive maintenance, report any equipment malfunction to supervisor.

  • Develop and implement quality assurance programs relating to calibration and maintenance of equipment.

  • Coordinate maintenance personnel, cleaning services, and vendor visits.

  • Develop safety, training courses for equipment operators.

  • Maintain all necessary supplies (ordering and stocking).

  • Maintain certification in Basic Cardiac Life Support procedures and be able to respond to emergencies.

  • Instruct and train students, nurses, or other allied health personnel as requested regarding the capabilities and limitations of the methodologies, the optimal utilization of tests, and the interpretation of the procedures.

  • Assure compliance with regulatory and legal requirements.

Education

ARDMS Certification as a Registered Vascular Technician (RVT).

Experience

One year of clinical experience in peripheral vascular imaging, two years preferred, and CPR certification or obtain certification within 90 days of employment. OR AN EQUIVALENT COMBINATION OF RELEVANT EDUCATION AND/OR EXPERIENCE

VIEW JOBS 12/5/2018 12:00:00 AM 2019-03-05T00:00 Position Summary The Vascular Surgery APP Hospitalist provides direct patient care services for the patients on the Vascular Surgery Service patients in Duke University Hospital in collaboration with current APAP vascular team, residents and surgeons. The APP performs physical exams, admissions, collects and documents data, conducts diagnostic and therapeutic procedures, orders and schedules laboratory studies act as a professional consultation, prescribes appropriate interventions and medications, coordinates the daily care and discharge of patients and provides direct patient care services. This is a inpatient hospitalist position and there is no OR opportunities with this position. The APP contributes to excellence in patient care, research, teaching, and provides leadership to the organization. Hours Primary hours will be Monday through Friday, including holidays, rotating 14 (fourteen) 12 hour shifts per month. Flexibility is required to meet clinical needs. Responsible to The Vascular Surgery Advanced Practice Provider will report directly to the Advanced Clinical Practice Team Leader and Director. The Vascular Surgery APP's clinical practice is supervised by the Vascular Surgery Supervising Physician or under the supervision of all back-up attending physicians listed in the Supervising / Collaborative Practice Agreement. In addition, s/he will be accountable for clinical outcomes and defined quality indicators as determined by the appropriate inpatient unit / Clinical Service Unit Balanced Score Card and to the standards set forth by the Department of Advanced Clinical Practice Job Responsibilities and Standards 1. Provide /coordinate clinical care for vascular surgery patients at Duke Hospital being followed in Duke Hospital and ER when on the primary or consulting vascular surgery service through established order sets and plans of care and under the supervision of the vascular surgeon who is either on site or available by pager. Standards * In collaboration with attending vascular surgeon(s) round daily on patients to assess, diagnose, plan treatment, set priorities and realistic outcomes, and evaluate the effectiveness and cost efficiency of cardiothoracic patient care; * Collaborate with the chief resident, residents and fellows to formulate treatment plans and monitor patient progress; v Performs the following skills: a. Patient history and physical exam b. Ordering and interpretation of laboratory, radiology, and/or diagnostic procedures/tests c. Airway and Tracheostomy management d. Evaluation and management of chest tube and drainage systems e. Order and administer infusion therapy f. Order blood products g. Order / Manage enteral nutrition h. Manage support devices, including (but not limited to) hemovac, NG tubes, G tubes, J tubes, JP drains, wound vac i. Manage acute cardio and/or respiratory decompensation, including rapid response and/or code v May order and/or perform the following procedures: Please note: A signed competency sheet will be submitted once training is complete. a. Blood sampling via central venous lines including: * Arterial and venous blood gasses * Venous line removal * Soft tissue drain removal * Internal and external hemorrhage control i. Wound management * Wound vac * Application of dressings and bandages * Simple debridements. * Suture placement * Local cauterization * Suture / Staple removal f. Place nasogastric feeding tubes, including postpyloric g. Remove temporary dialysis catheters and arterial and venous sheaths h. Defibrillation / cardioversion * Assess patients for change in status and institute appropriate interventions. Initiate emergency interventions in an effort to stabilize the patient until physician support arrives. * Conduct preoperative evaluation; including (but not limited to) establishing diagnosis, and baseline status * Conduct postoperative evaluation, stabilization and management. * Develop patient care pathways, protocols and guidelines for the management of specific problems in conjunction with physicians and other members of the health care team. * Analyze clinical practice patterns so as to provide the best medical care and to increase effectiveness and efficiency. * Communicate verbally and in writing all pertinent patient information to supervising MD, nursing staff, patient resource manager, home health agency, and other relevant healthcare providers. * Document in the patients' chart all relevant data including but not limited to results of diagnostic tests; laboratory results; patients' condition and response to therapies/interventions; communications with supervising MD. * Use prescriptive authority for prescribing, ordering, and or administering drugs; including (but not limited to) schedule II, III, IV, and V drugs and necessary medical devices. 2. Educator Standards: * Assess educational needs of the vascular surgery service patient-care staff rgery service patients / families and provide patient/family education interventions as indicated. Document all pre and post-op education in patients' medical records. * Implement and evaluate appropriate educational programs for healthcare providers as appropriate. * Role model and precept healthcare learners through contractual agreements. 3. Research Utilization and Investigation Standards: * Utilize current research and evidence-based decision-making in all clinical practice. * Incorporate clinical research findings in the development and implementation of standards of care. * Support IRB approved clinical research/trialstherapeutic interventions * Participate in measuring and documenting outcomes. 4. Consultant Standards: * Provide expertise and resource information to other healthcare providers, patients and families, and the community at large related to vascular diseases, procedures, interventions, and conditions. * Develop, implement and evaluate standards of care/practice guidelines/policies and procedures/care maps/protocols within area of specialization. 5. Professional Leadership and Development Standards: * Role model advanced practice professionalism * Participate in at least one professional organization * Enhance the body of knowledge in area of specialization through written publications, oral presentations, posters, continuing education, etc. * Maintain all professional requirements for licensure and certification.
