Diversicare operates 76 centers in ten states primarily in the Midwest, Southeast and Southwest. As Diversicare team member, you will play an instrumental role in the care of our patients and residents, our most important responsibility.
We are seeking compassionate team members to assist our patients and residents with activities of daily living. 1.Assure the safety of each resident when transporting to and from the doctor's office and/or hospital.2. Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.3. Observe the following guidelines at each visit: a. Sign in resident upon entrance. b. Present resident's documents to office personnel.4. Before leaving doctor's office, please observe the following: a. Obtain resident's documents. b. Obtain any new prescriptions. c. Check to make sure doctor has written on the doctor's order form. d. If follow-up appointment necessary, obtain appointment cards.5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.6. Performs other duties as required and assigned.
Qualifications Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.Possess a valid state driver's license.Qualified as a Certified Nurse Assistant (C.N.A.).Clean driving record.Clean criminal record.Any other state required certifications
