Ambercare Home Health, an Addus HomeCare Company is an industry leader and currently searching for PRN Transitional Care Coordinator - Licensed Practical Nurse for our Value Based Program in Tucson, AZ.

What You'll Do:

  • Receive incoming calls/referrals for clients enrolling in value-based programs and maintain accurate lists of enrolled clients

  • Coordinate or perform an initial in home visit with enrolled clients

  • Ensure completion of a medication reconciliation for all enrolled clients

  • Perform telephonic or in person training to clients on their medical conditions, medications and care plans

  • Work with client to identify gaps in in-home care and refer client for home health, hospice or in home physician services as needed.

  • Identify additional local resources to assist client with clinical and social issues such as local paramedical services, urgent care centers, non-emergent transportation, pharmacy resources, etc.

  • Develop relationships with extended services and refer clients as necessary

  • Receive and document Change in Condition calls from the health engagement team and direct care staff

  • Review and verify Change in Condition reports from Electronic Visit Verification and other electronic systems, perform follow up as necessary

  • Monitor internal processes for efficiency and validity pre & post product launch/changes.

  • Manage data flow from operations and report outcomes to internal and external customers

Qualifications:

  • Current RN or LPN license with at least three (3) years of experience in a clinical setting

  • Previous Experience in home care and home health. Knowledge of managed home healthcare is preferred

  • Possess and maintain valid CPR certification.

  • Candidate must possess excellent verbal and written skills

  • Solid computer proficiency in Microsoft Office including Excel and web based application

  • Committed to exceptional customer service and meeting customer needs

  • Ability to complete tasks in a multi-phased, fast-paced environment

  • Must have reliable transportation, current driver's license, and appropriate automobile insurance
