The "care coordination" process involves a high level of interaction with community health providers, organizations, agencies, Medicaid/Medicare beneficiaries, and individuals and families in need. The goal is to support optimal care needs in place-based settings, make referrals, help schedule appointments, provide information/referrals and document relevant health data via a secure web-based system, as well as complete necessary follow-up to ensure continuity of care.

AmeriCorps members do not provide health related services to prohibited items. The will help those in need navigate to appropriate community-based service providers, state agencies, etc. and eliminate barriers to care. Members will provide peer-to-peer training for other members, manage and educate community on resources related to available health and human services, behavioral and social-emotional (mental) health, addition/substance abuse, food, and other services that address the Social Determinates of Health (SDoH) and provide, and advocacy.

Members will develop and engage in community outreach strategies, workshops, events, activities, handle telephone inquiries and cases, document information, referrals and case information in appropriate tools, network with community-based service providers to simplify access to services. Members will also help to maintain and expand the Delaware 2-1-1 database by identifying new and updating existing resources.
