Summary

Responsible for directing the entire operation of a nursing unit in a long term facility

Qualifications

  • Education: Graduate of accredited school of nursing, degree preferred.

  • Licenses/Certification

  • Licensed as a RN (if state requires) or LPN/LVN in state employed.

  • Valid CPR teaching certificate, if applicable.

  • Experience: Three years of nursing experience, of which one year was in a long term care environment.

Essential Functions

  • Performs General Management Functions such as hiring, disciplining and evaluating employees

  • Plans and facilitates meetings and committees to address resident care issues for the unit

  • Manages area of responsibility with the goal of achieving and maintaining the highest quality of care possible.

  • Participates in developing, implementing and evaluating programs that promote the recruitment, retention, development and continuing education of nursing staff members.

  • Contacts physicians as necessary and ensures physicians interventions are timely and appropriate

  • Reviews staffing patterns and census of nursing units, and reassigns personnel when necessary to ensure staffing meets resident needs and budgetary controls.

  • Monitors the clinical operations of the unit and resident's conditions and ensures that appropriate and quality care is administered.

  • Obtains medications, supplies and medical records needed to provide safe, efficient and therapeutic care to residents on a continuing basis.

  • Assists in the orientation of new personnel, monitors their skills and guides and observes staff that may need assistance with procedures.

  • Participates in facility "QA" program

  • Performs other tasks as assigned.

Knowledge/Skills/Abilities

  • Skilled in motivation of staff and leadership.

  • Skilled in work assessment.

  • Ability to communicate effectively with residents and their family members, and at all levels of the organization.

  • Knowledge of state and federal requirements for accreditation and certification.

  • Possesses basic computer skills

  • Ability to work with a culturally diverse resident and employee population.

The Care Manager will assist the physician with patient calls, maintaining and documenting in the patient medical record, coordinate patient care with other health care providers. The Care Manager will focus on prevention, health promotion, patient and office team education, and patient assistance. Under the supervision of a physician the Care Manager is involved in rendering medical care such as Medicare wellness visits, diseases as well as advise, counsel and instruct patients on health care treatment and prevention. The Care Manager serves as a patient advocate/liaison, overseeing the continuum of care and chronic disease care management for the patient. The Care Manager will take a proactive role by early intervention and collaborative practice with staff nurses, medical assistants, physicians, office administrative team, ancillary services, payer sources, and outside agencies assuring appropriate use of resources. The Care Manager will focus on positive quality outcomes and streamlining the patient care delivery process while avoiding duplication and fragmentation of patient care through hands-on care management of patients. The Care Manager will work with the Practice Manager and Clinical Coordinator for staff education and development of ongoing quality measure work. The Care Manager will work, with payers and gatekeepers to assure necessary pre-certifications / authorizations are obtained. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area. Minimum Education/Training: Bachelor#s Degree in Nursing; consideration may be given to experienced candidates actively pursuing a Bachelor#s Degree in Nursing. Consideration#may be given#to candidates with these certifications: Certified Diabetic Educator (CDE) or Certified Asthma Educator (CAE). Minimum of three years of experience in clinical practice with a focus on education, quality improvement processes/programs, and regulatory agency compliance. Clinical experience in a patient care setting. Care Management experience. Formal Training in Case / Care Management or must complete required training within 60 days of hire. Training may be required by certain payers as part of pilot programs. Registration/Certification or License Required: RN licensure in the State of Michigan. Current BLS for the Healthcare Provider Certification (or completed within 60 days of hire). Preferred Education: Accredited Care Manager (ACM). Preferred Experience: Five years experience in clinical practice with a focus on education, quality improvement processes/programs, and regulatory compliance. Physical Requirements: Frequent sitting.
