1.Receives report on patient/resident status from previous shift, evaluates and ensures resources in place to meet needs2. Makes physical rounds assessing quality of care, intervening with provision of care and interacting with care givers and patients/residents3.

Provides leadership and support with care in carrying out their responsibility in a professional manner to include but not limited to: timely medication passes, required care plans as directed by physician and/or Registered Nurse, documentation and treatments.4. Evaluates performance of direct care team members and informs Director of Nursing Services if there are any coaching or corrective action opportunities. Assists with investigation as needed or required.5.

Evaluates and monitors to verify patient/residents' needs are met and policies and procedures are followed.6. Evaluates critically ill residents; reports findings to physician and records observations; may provide direct care as needed. Notifies Director of Nursing Services and Administrator according to policy and practice.7.

Responds and coordinates actions in emergency situations notifying the Director of Nursing Services and Administrator according to policy and procedure.8. Works with Director of Nursing Services to facilitate solutions to any systemic or procedural concerns.9. Ensures safe work practices are followed.10.

Provides empathetic communication to families, patient/residents regarding change of condition and in response to care concerns.11. Ensures resident rights are respected.12. Works directly with care team members to assure care is provided.13. Other duties as assigned by Director of Nursing Services and/or Administrator.


1.Current license by the State Board of Nursing as a Registered Nurse with experience in the long-term care industry preferred; at least three years nursing experience required.2. Ability to plan, evaluate and coordinate services.3. Experience in triaging tasks, supervision of nursing team members and coordination of services to provide the optimum resident and patient experience.

Consults with residents' physician in planning resident care. Periodically reviews and updates comprehensive care plan.</p><p>2.Works with interdisciplinary team members in developing a discharge plan for individual residents and assists in achieving discharge planning.</p><p>3.Communicates and acts as a liaison among residents, family, physicians, and staff.</p><p>4.Conducts resident rounds to assess and/or supervise nursing care, assess condition of residents, and safe, clean, comfortable environment.</p><p>5.Audits staff nurses to ensure that all resident care systems and processes are followed per policy.</p><p>6.Reviews all closed charts of expired resident and ensures appropriate documentation by nursing during the last 48 hours and discharge orders by physicians are enclosed (to contain how and where discharge - i.e. to funeral home via stretcher). Reviews discharge summaries of all residents.</p><p>7.Conducts periodic reviews of Care Plans to ensure they are interdisciplinary and updated by the Charge Nurse as changes occur.</p><p>QUALIFICATIONS</p><p>SUMMARY OF QUALIFICATIONS:</p><p>1.Maintains a current, valid License as nurse in good standing.</p><p>2.Possesses a minimum one (1) year of experience in nursing service administration. Additional education or experience in such areas as rehabilitative or geriatric nursing is preferred.</p><p>3.Possesses strong knowledge of state, federal and local regulations as they pertain to long term care.</p><p>4.Possesses good communication skills and is patient and self-disciplined.</p><p>5.Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.</p><br> Birmingham Nursing And Rehabilitation Center, LLC Birmingham AL
