Position Purpose:

The primary purpose of this position is to direct, oversee, and monitor the nursing services provided on a specific unit. The Care Team Manager shall support, educate, evaluate and provide clinical expertise to Health Services. This position also assists and may, as needed perform the day to day nursing activities on that unit and perhaps throughout the facility. All duties will be performed according to local, state, and federal rules, regulations and guidelines that govern our state to ensure that the highest quality of resident care is delivered and maintained at all times.

Essential Functions/Accountabilities:

  • Assists the Director of Nursing in directing the day-to-day functions of nursing activities in accordance with current rules, regulations and guidelines that govern long-term care as evidenced by documentation and/or observation.

  • Creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit.

  • Provides leadership to and manages the operations of the assigned unit including but not limited to ensuring that: all nursing service personnel comply with the facility's policies and procedures including dress code, infection control and safety procedures; all nursing service personnel are in compliance with their respectful job descriptions; adequate stock levels of medications, medical supplies and equipment is maintained; attendance and tardiness is monitored; staffing levels are sufficient to maintain quality of; work assignments are developed; team member performance evaluations are completed in a timely manner.

  • Ensures that all licensed nurses comply with HPHC's written procedures for the administration, storage and control of medications and supplies. Monitors medication passes and treatments, and reviews medication administration records for completeness, accuracy and adherence to stop order policies.

  • Makes daily rounds of the nursing unit to ensure that all nursing service team members are performing their work assignments in accordance with acceptable nursing standards; monitors nursing care to ensure that all residents are treated fairly and with kindness, dignity and respect; reviews nurses' notes to ensure that they are informative and descriptive for the nursing care being provided and that they reflect that the care plan is being followed and the residents' response to care.

  • Assists with the development, implementation and maintenance of an effective orientation program for staff; participates in developing, planning, and scheduling of in-services for staff training.

  • Ensures that the nursing service work areas and all resident care rooms, treatment and common areas are maintained in a clean, safe and sanitary manner. Reports all occupational exposures to blood, body fluids, infection materials and hazardous chemicals and any other work injuries in accordance with the facilities policies and procedures governing accidents and incidents.

  • Makes written and oral reports/recommendations to the Director of Nursing as necessary/required, concerning the operation of the nursing service department.

  • Participates in the implementation of the facility's quality assurance program and in various committees within the facility (i.e. Level of Living, Utilization Review, Care Team Managers, etc) and in Level of Living reviews. Participates in facility surveys made by authorized government agencies as requested by the Administrator or Director of Nursing.

  • Assists in developing methods for coordinating nursing services with other resident services to ensure continuity of the residents' total regimen of care.

  • Provides direct nursing care as necessary; starts IVs, obtain sputum, urine, and other specimens for labs; monitors and obtains vital signs.

  • Authorizes the use of restraints when necessary and in accordance with the facility's policies and procedures.

  • Keeps the Inter-Disciplinary team informed of the status of residents. Participates with other Inter-Disciplinary Team members, the residents and their family members in the care conference process to develop, implement, evaluate and update the residents' comprehensive plan of care.

  • Assists in the admission, transfer and discharge of residents as needed.

  • Assists with the development, implementation, and evaluation of residents' acute care plans. Reviews resident care plans for appropriate resident goals, problems, interventions and revisions based on nursing needs. Assists in the evaluation of each resident's physical and emotional status. Consults with the residents, family members, and physicians in planning the resident's care, treatment and rehabilitation as necessary.

  • Assists the MDS team in planning, scheduling and revising the MDS.

  • Reviews residents' medical and nursing treatments to ensure that they are provided in accordance with the residents' care plans and wishes.

  • Reports and assists in the investigation of any abuse and/or misappropriation of resident property as evidenced by documentation. Reviews complaints and grievances made by a resident and makes a written/oral report indicating the actions taken to resolve the issues.

Other Responsibilities:

  • Provides facility supervision for nursing care.

  • Participates in various organizational committees as requested by the Director of Nursing.

  • Educates residents and their families to nursing care and other related services as necessary.

  • Attends and participates in continuing education programs designed to keep current with changes in the nursing profession, as well as to maintain licensure requirements.

Requirements:

  • 2 years of experience as a supervisor and/or clinical manager in a hospital, skilled nursing care facility, or other related medical facility.

  • Must possess a Nursing Degree from an accredited college or university, possess a current, unencumbered, active license to practice as an LPN or RN in the State of Ohio.

Minimum Qualifications:

  • Must be able to read, write, speak, and understand the English language. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

  • Must possess the ability to make independent decisions when circumstances warrant such action.

  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.

  • Must possess leadership and supervisory ability and be willing to work harmoniously with and supervise other personnel.

  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.

  • Must not pose a direct threat to the health and safety of other individuals in the workplace.

  • Must be able to move intermittently throughout the work day.

  • Must be able to cope with the mental and emotional stress of the position.

  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.

  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.

  • May be necessary to assist in the evacuation of residents during emergency situations.

Working Conditions:

  • Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses? stations, resident rooms, etc.)

  • Moves intermittently during working hours.

  • Is subject to frequent interruptions.

  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.

  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.

  • Communicates with the medical staff, nursing personnel, and other department supervisors.

  • Works beyond normal working hours, and in other positions temporarily, when necessary.

  • Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).

  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants, and other air contaminants.

  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.

  • May be subject to the handling of and exposure to hazardous chemicals.

  • Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

The employee is the provider of hands on resident care, including, but not limited to, all activities of daily living, planning, assessing, evaluating and providing quality nursing care. Participates in in-service education and disaster preparedness.</p><p>ORGANIZATIONAL RELATIONSHIP</p><p>This position is supervised by the Director of Assisted Living or his/her designee. Ultimate direction is provided by Administrator/Executive Director.</p><p>DUTIES AND RESPONSIBILITIES</p><p>Essential Job Functions:</p><ul><li><p>Provides assistance, as needed, to residents in all aspects of activities of daily living, including but not limited to bathing, showering, grooming, dressing, toileting, dining (both feeding and hydrating for regular meals and snacks), transferring residents from bed into wheelchair and back, transferring from wheelchair to toilet and back, positioning residents and ambulation.</p></li><li><p>Upon successful completion of Medication Administration training, may pass medications in accordance with physician's orders. Responsible for proper recording of dispensing of medications.</p></li><li><p>Assists in resident needs assessment and safety by observing and reporting all changes or unusual findings in, but not limited to, physical condition, attitude, psycho-social reactions and appetite to charge nurse. Stands or walks 90% of the work shift. Must be capable of bending, squatting and lifting heavy residents. Must be capable of pushing resident in wheelchairs (approximately 250 lbs.) within the community. Procedures for lifting and transferring residents must be followed exactly to prevent injury to the resident and/or employee. Works indoors majority of the time, however, may participate in outdoor functions and walks with residents. May be exposed to offensive odors. May be dealing with confused, combative residents.</p><p>Safety: Considerable responsibility for safety of others, including residents and staff members. The possibility of causing an injury to others is considerable. Regular ability to understand and to follow policy and procedures of fire &amp; life safety, infection control, disaster preparedness, safety rules and regulations and department rules.</p><p>Working</p><p>Conditions: May be exposed to infection, odors and to peculiarities in behavior and reactions of the aged.
