Weekend RN/LPN Supervisor

Job Summary

Provides, plans, coordinates or manages nursing care and health education to nursing home Residents on assigned unit.

Qualifications:

  • Must have a current license to practice as a Registered Nurse (RN) preferred, or Licensed Practical Nurse (LPN). Current CPR certification is desired.

  • A minimum of two years experience in long term care setting. Prefer prior management experience.

  • Must have excellent assessment, planning, organization, communication and interpersonal skills with an understanding of person centered care.

  • Ability to work flexible shifts including periodic weekends, holidays and off shifts.

  • Ability to read, analyze and interpret, professional journals, technical procedures or governmental regulations. Ability to communicate orally and through written reports and other documents relating to resident, staff and others. Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers, and the public.

  • Ability to define and solve problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical journals and deal with several abstract and concrete variables.

  • Must be able to express self clearly and concisely in written and oral communication and to communicate effectively in an interdisciplinary care setting with residents, families, and staff members.

Essential Functions and ResponsibilitiesCare and Services:

  • Demonstrates the knowledge and clinical expertise required to effectively supervise all nursing activities in the facility or the units to which assigned.

  • Reviews and implements all nursing procedures and systems relative to the unit (s).

  • Assesses and evaluates the systems which facilitate the delivery of quality Resident care.

  • Assesses and evaluates the performance and competency of staff on unit(s).

  • Monitors documentation daily, audits Medication records, treatment records, and nurses notes to assure treatments are rendered accurately and documentation reflects current resident needs and status.

  • Provides one to one education to nursing staff as needed to improve level of care, documentation, and adherence to state and federal regulations.

  • Conducts rounds several times per shift to evaluate the quality of care provided, identify changes in condition, and assist residents and staff as needed to address care or service needs.

  • Reviews 24 hour report at the beginning of each shift, assesses residents with changes in condition, and follows up with physicians as needed to address concerns.

  • Spot checks nurses notes against 24 hour report to assure proper documentation and follow up occur. Utilizes the "pink slips" to identify changes in condition and resident needs.

  • Uses a systematic approach to provide individualized nursing care.

  • Performs nursing assessments regarding the health status of the Resident.

  • Contributes to the Resident's assessment (MDS/RAPs) and the development of a plan of care.

  • Makes nursing diagnoses, which serve as the basis for the strategy of care.

  • Develops a plan of care based on assessment, implementing nursing care.

  • Evaluates the Resident's responses to nursing interventions.

  • Selects and institutes appropriate nursing intervention, which might be required to stabilize a Resident's condition and/or prevent complications.
