Nurse Practitioner or Physician Assistant, Urgent Care (acute primary care only)University Medical Partners
Castro Valley and Pleasanton, CA University Medical Partners (UMP), a partner of Stanford Medicine, is seeking experienced full-time BC/BE Nurse Practitioner and/or Physician Assistant to join our Walk-In Clinic located in Castro Valley, CA, and our Primary Care Clinics located in Pleasanton, CA

Position Details:

Minimum of 2 years Primary Care or Urgent Care experience required
Express care - Castro Valley Express Care and surrounding primary care clinics including Pleasanton/Gibraltar clinic every other Saturday from 9 1
Must be comfortable working independently
Clinicians will typically see urgent care cases and some patients, from our primary care office, with chronic conditions with an acute exacerbation symptoms
Ability to see pediatric patients over 6 months
Clinicians will also provide acute care telehealth visits across the network as needed
Clinic hours are Monday-Friday 7am-7pm, Saturday and Sunday from 9am-7pm (shift-based)
Flexible work schedule, must be able to work some nights and weekends
No primary care patient panel responsibilities but will help with inbasket work periodically
State-of-the-art clinic include on-site imaging, labs, and dedicated procedure rooms
Easy in office access to primary care, pediatrics, specialists, and ancillary services

VIEW JOBS Organization: Bay Administration Position Overview: Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience. This position works in collaboration with the Physician, Utilization Manager, Medical Social Worker and bedside RN to assure the timely progression and transition of patients to the appropriate level of care to prevent unnecessary admissions or readmissions. The Care Management process encompasses communication and facilitates care across the continuum through effective resource coordination. The goals of this role are to include the achievement of optimal health, access to care, and appropriate utilization of resources balanced with the patients' self -determination while coordinating in a timely and integrated fashion. He/She collaborates with patients, families, physicians, the interdisciplinary team, nursing management, quality, ancillary services, third party payers and review agencies, claims and finance departments, Medical Directors, and contracted providers and community resources. If assigned to the Emergency Department, the Care Management process is to address complex clinical and social situations efficiently in order to avoid unnecessary admissions.
