  • We are currently seeking part time or full time New York State (RN) Registered Nurse to asses and complete (UAS) Uniform Assessments in Brooklyn, Bronx, Manhattan, Queens, Long Island, Yonkers, White Plains and more!

  • Must be UAS certified

  • Spanish-speaking a plus
  • Willing to train in UAS, if you have at least one year of Nursing Experience in hospital setting.

    Qualifications:

    R.N. diploma from an accredited NLN program with a minimum of an Associate's Degree (BSN preferred). Current, valid and unencumbered RN license, certificate or registration or credentialing as required
    RN working in a home care environment or in an acute, sub-acute, or long term care (LTC) setting or Managed long term care (MLTCP)
    Must satisfactorily pass the NYSDOH required UAS training and any updated training modules
    UAS Certified preferred
    Intermediate ability to use computerized systems such as Microsoft Outlook, Microsoft Word, Excel.
    Travel by public transportation or private vehicle is required. If the employee drives their own vehicle, they must have a valid, unencumbered driver's license issued by the state in which they work and a satisfactory driving record and proof of vehicle insurance
    Bilingual English-Spanish (other languages will be considered)

    Required experience:
    1 year of strong Nursing experience in a hospital setting.
    2-year Home Care experience a must
    Excellent Patient Care and Assessment Skills
    The ability to use computerized systems such as Microsoft Outlook, Microsoft Word, Excel, and to navigate the DOH UAS health care system proficiently.
    Key functions include:
    Conducts UAS Assessments at member's place of residence
    Completes the UAS, Aide Task Service Plan and all other documents required by health plan
    Conducts validation audits of UAS and ATSP and uploads UAS to DOH website
    Communicates daily with office staff on all required updated information for all assigned members in the required timeframe
    Provides member education regarding health issues, medication and plan benefits



    Requirements:
  • Nursing (RN) license
  • At least 1 - 2 years of field-based visiting nurse experience, UAS experience preferred (with 2 weeks training/ orientation)
  • Experience in Home/Community based or Long-Term Care services delivery
  • Current and valid NY drivers license or ID
  • UAS Training offered
    (3 weeks paid training)
    - 2 Years Home Care Experience
    - Are you UAS Certified?
    - Computer Savvy/ Skilled (a must)
    - Available Full Time
    - Conflict of Interest Free

    Benefits

    Regular Rates:

    $46/Hour

    Full Time Position. Mon-Fri. 9-5. 35-38 Hours/Week.

    2 Assessments Daily

    3 Hours Per Assessments + 1 Hour Travel Time

    * Conflict of Interest Policy must be signed prior to starting

    Note: not Currently working with MLTC, CHHA or LHCSA

    * Simple Work / No Paperwork @ Home

    Equipment will be provided: laptop/I-Phone/MiFi
