The Division of Fee for Service Management is looking for a highly motivated individual to join our team as a Tribal ALTCS Administrator. The selected candidate is responsible to develop and maintain programs/initiatives/processes for care coordination, case management, disease management, behavioral health services, home and community-based services expansion of services and healthcare education for the AHCCCS population of American Indians/Alaska Natives (AI/AN) FFS members, working with tribes, facilities and members. This will be accomplished through an ongoing collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the members' health needs. It involves advocacy, communication, resource management, and promotes quality and cost-effective interventions and outcomes. Major duties and responsibilities include but are not limited to:
Maintains active oversight of the Tribal ALTCS Case Management program/contracts, and the care coordination and Behavioral Health initiatives as they relate to the overall care coordination efforts of the AI Health Program and Tribal ALTCS members.
Supervises, directs activities, and monitors/evaluates the staff members working with Tribal ALTCS Case Management program/contracts.
Develops programs/initiatives/processes for care coordination, case management, disease management, behavioral health services, home and community-based services and health care education for American Indians/Alaska Natives (AI/AN) FFS members.
Oversees development and maintenance of policy related to case management contracts, and other care coordination activities.
Represents DFSM/AHCCCS at meetings within the agency and externally.
Travels to remote areas of Arizona, as needed, when collaborating with AI/AN tribes and facilities.
Serves on the DFSM Leadership Team.
KNOWLEDGE, SKILLS AND ABILITIES (KSAs):
Knowledge of AHCCCS, ALTCS, Medicaid and Medicare Federal Regulations.
Supervisory experience, strong leadership and organizational skills to handle multiple projects and responsibilities and outstanding analytical skills to identify issues and present recommendations..
Demonstrated experience working with Indian Health Services (IHS) and tribally operated 638 facilities.
Superior communication skills, both verbal and written.
Cultural and interpersonal skills in working with people of diverse cultures and socioeconomic backgrounds, including AI/AN tribes.
3 - 5 years of experience with care coordination and case management in working with the AI/AN population.
A Master's Degree in health or a health related field with a focus on care coordination for the AI/AN population.
Familiarity with health care delivery to AI/AN.
Tribal Member preferred.
