The Inpatient Specialty Program, Care Manager /Triage Nurse (RN) provides initial triage assessment of inpatient referrals to the ISP hospitalist service and helps to assess the reason for referral, appropriateness of referral, urgency and initial needs. This RN will work with referring clinicians/services to ensure accurate initial assessment information is captured and communicated to the admitting hospitalist. They will also work with the hospitalist group leadership to manage the department's overall triage workflow during the day and be a primary resource/contact for referring clinicians. The ISP triage RN must possess strong communication skills, clinical acumen, critical thinking, thoughtful decision-making and organizing abilities, as well as interpersonal skills to facilitate teamwork and to support the ISP team and clinical staff.
Conducts initial triage nursing assessments of referrals to ISP in advance of admission
Identifies appropriate patients for admission & shares pre-admit information with the admitting physician
Assists with creating the daily team rounding lists
Assists with schedule changes & shift tracking
Identifies priorities for patient care management
Reports concerns or urgent messages to the physician team
Enacts problem-solving and critical thinking skills

Facilitates smooth transitions between care environments
Provides effective patient/family/staff teaching
Utilizes resources effectively and efficiently

CA-RN license required.
BLS required from American Heart Association.
2-5 years clinical experience in medical, surgical or critical care. Experience in project management/process improvement preferred. Candidate should also have knowledge of inpatient hospital operations and possess excellent clinical assessment skills. Other requirements include the ability to interface effectively with patients, providers, management and employees of the organization as well as facilities and agencies in the community. This position requires an individual who has strong communication skills, is self-directed, responsible, able to work in diverse groups, exhibits excellent time management skills and works autonomously. PC and strong writing skills, ability to teach others and develop program materials required.
