Job Summary:

  • The Treatment in Motion (TIM) Case Manager role is part of the Treatment in Motion program team (and in larger part the ETS Behavioral Health program) which supports ETS' mission by developing trusting relationships with T.I.M. program participants who explore their goals and receive ongoing support to engage in services that will help improve their stability and quality of life.

  • The TIM Case Manager role makes a critical difference at ETS by providing outreach, engagement, case management and care coordination services to adults experiencing homelessness and behavioral health challenges who have a diagnosis of opioid use disorder. The emphasis is on serving individuals whose complex needs are a barrier to engagement in Medication for opioid use disorder (MOUD) treatment .

  • The position is full-time onsite, the location is at ETS' Seattle Clinic, ETS' Seattle Mobile Medication Unit, and other outreach locations in metro Seattle area.

  • The pay range for this position is $28.00 per hour - $30 per hour. (The annual pay equivalent is $58,240 - $62,400 per year.) ETS strives to achieve pay equity and transparency as part of our goal to create an equitable workplace.

  • The hours are 5:00 AM - 1:30 PM, Monday through Friday with rotating Saturday shifts.

What you contribute to this role

  • Responsibilities:
  • Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.

  • Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.

  • Provide outreach, engagement, intensive case management and care coordination services for assigned participants:

  • Meet participants where they are, either on the streets or at other service facilities (including jail), to explain offered services and establish a working relationship with the goal of assisting participants into MOUD treatment through TIM program.

  • Develop rapport and a trusting relationship with participants through respectful outreach and participant-centered engagement practices.

  • Develop and implement in collaboration with the participant, an individualized service plan (ISP) which addresses the participant's stated needs and priorities. Review and update this plan regularly to reflect progress or attainment of articulated goals and the emergence of new participant needs and to help the participant move toward the achievement of autonomy.

  • Assist participant in developing plans for regular attendance in treatment activities as determined though the ISP.

  • Assist participants in obtaining housing and maintaining housing stability when possible.

  • Assist participants with establishing and maintaining primary medical care and addressing chronic health conditions.

  • Assess nature and severity of participants' substance use issues and match clinical interventions to participants stage of change, focusing on reducing the harmful consequences of substance use through self-change methods.

  • Address participants' needs for mental health and substance use disorder treatment services and facilitate engagement in appropriate supportive services.

  • Assist participants in achieving financial stability through access to entitlement programs, protective payee-ships, educational and vocational programs, and employment opportunities.

  • Develop and maintain productive working relationships with community programs which may support participants to provide effective coordination of care.

  • Provide services consistent with ETS/REACH program policies and procedures.

  • Assist participants in keeping appointments.

  • Assist participants in developing a spending plan and in shopping.

  • Advocate for the Participant with a wide variety of other service providers:

  • Assist with accessing needed services by setting up and attending appointments with participants to promote continuity of care.

  • Identify gaps and barriers in available community resources and advocate for systemic changes.

  • Attend TIM team meetings, and other required meetings.

  • Utilize clinical supervision, psychiatric consultation, consultation with SUD counseling, and peer support for exploring clinical intervention strategies, countertransference, resource development, self-care and burnout prevention.

  • Develop and maintain Participant files for assigned caseload according to agency, contract, and state requirements.

  • Local travel on behalf of the agency is a job requirement.

  • Assist with driving program vehicle(s) and assisting with site-based safety when necessary.

  • Provide insight and assist with program development on ongoing quality improvement to ensure the highest quality participant experience.

Requirements

What you bring to this role

  • Qualifications:

Education and/or Relevant and Lived Experience:

  • High School Diploma required

  • Undergraduate degree preferred.

  • Academic training in social service field and substance use disorder and mental health treatment desired.

  • SUDP/SUDPT and/or MSW highly desirable.

  • A minimum of two years of experience in work related to substance use disorders, social work, or outreach programs.

  • Individuals with lived experience related to our patient population highly desirable.

  • Experience providing services from a harm reduction perspective to individuals experiencing behavioral health challenges highly desirable.

Knowledge and Skills:

  • An understanding of racial justice and social equity and a commitment to helping create an equitable environment for all ETS clients and patients as well as fellow staff.

  • Ability to establish and maintain effective working relationships with clients, patients, and staff from a wide variety of ethnic, socioeconomic, and cultural backgrounds.

  • Possession of a valid driver's license.

  • Strong interpersonal skills and verbal/ written communication skills.

  • Excellent organizational skills and ability to prioritize workload, work independently, and complete tasks timely and efficiently.

  • Dependable, able to work under pressure, receptive to change, willingness to learn, cooperative approach to problem solving.

  • Flexible team player, with excellent attention to detail.

  • Ability to maintain confidentiality and use discretion when handling highly sensitive information.

  • Ability to set boundaries, resolve conflict and de-escalate issues.

  • Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.

Additional Essential Information:

Physical Requirements:

  • The employee may be exposed to illicit drug residues and fumes or other bio-hazardous materials when carrying out job functions. There is also potential for exposure to bloodborne pathogens. ETS will provide employees with appropriate training to limit the risk of exposure to bloodborne pathogens. Policies and procedures are in place addressing each item specifically.

  • The employee is regularly required to be on their feet: standing, walking, and kneeling; occasionally to climb balance, stoop or crouch and crawl. They will regularly be in communicating with program participants while coordinating in and around clinic spaces (inside and outside). In addition, the position frequently requires the employee to work outside in inclement weather. Additionally, the employees is required to sit; use hands to finger, handle or feel objects, tools or controls; reach with hands and arms and talk or hear.

  • The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color, and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
