Traveling MDS Coordinator

Purpose of Your Job Position:

As a Consulate Health Care Traveling MDS Coordinator, you are entrusted with the responsibility of caring for ourresidents, families, co-workers, visitors and all others; as well as demonstrating in all interactions, Consulate Health Care's five core values of Compassion, Honesty, Integrity, Respect and Passion. The primary purpose of your job position is to coordinate the entire RAI process assuring the accuracy, timeliness and completion of the MDS, CAAs and interdisciplinary Care plan for all residents in the facility in accordance with all federal and state, certification guidelines. The Traveling MDS Coordinator monitors and documents the management of the Medicare and Managed Care residents in collaboration with facility team members.

Job Functions :

As the Traveling MDS Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. No supervisory function.

This job description is not a contract for employment, and either you or the employer may terminate employment at any time, for anyreason. Duties and Responsibilities of Traveling MDS Coordinator :

Complete accurate and timely MDSs, CAAs and plans of care in accordance with the RAI guidelines for all residents in the facility. Complete required documentation in an accurate and timely manner.

Provide resident status updates as needed to appropriate personnel. Attend and participate in department/facility meetings, as required. Act in compliance with Consulate, regulatory and professional standards and guidelines.

Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency. Adheres to facility policies and procedures and participates in facility quality improvement and safety programs. Comply with all of the Duties and Responsibilities noted for the MDS position. Able to travel to facilities to support the MDS process as needed. Become a Mentor for MDS staff throughout Consulate Health Care.

Provide an environment for new MDS staff to excel in the position. Travels between multiple facilities. Extensive overnight travel. Perform other duties, as assigned.

Requirements for Traveling MDS Coordinator : Education

Must possess a valid license as a Registered Nurse/Licensed Practical Nurse in the state(s) of practice.


  • Must have, as a minimum, three (3) years of nursing experience in a setting serving the same age or type of resident population served by this facility.

  • Preferred to have two (2) years of experience as a MDS Coordinator.
