Purpose of Your Job Position:

As a Consulate Health Care Traveling MDS Coordinator, you are entrusted with the responsibility of caring for ourresidents, families, co-workers, visitors and all others; as well as demonstrating in all interactions, Consulate Health Care's five core values of Compassion, Honesty, Integrity, Respect and Passion. You are entrusted to provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results. The primary purpose of your job position is to coordinate the entire RAI process assuring the accuracy, timeliness and completion of the MDS, CAAs and interdisciplinary Care plan for all residents in the facility in accordance with all federal and state, certification guidelines. The Traveling MDS Coordinator monitors and documents the management of the Medicare and Managed Care residents in collaboration with facility team members.

Job Functions :

As the Traveling MDS Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. No supervisory function. This job description is not a contract for employment, and either you or the employer may terminate employment at any time, for anyreason. Duties and Responsibilities of Traveling MDS Coordinator :

Complete accurate and timely MDSs, CAAs and plans of care in accordance with the RAI guidelines for all residents in the facility. Complete required documentation in an accurate and timely manner. Provide resident status updates as needed to appropriate personnel. Attend and participate in department/facility meetings, as required. Act in compliance with Consulate, regulatory and professional standards and guidelines. Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical currency. Adheres to facility policies and procedures and participates in facility quality improvement and safety programs. Able to travel to facilities to support the MDS process as needed. Become a Mentor for MDS staff throughout Consulate Health Care. Provide an environment for new MDS staff to excel in the position. Travels between multiple facilities. Extensive overnight travel. Perform other duties, as assigned.

Resident Rights :

Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. Ensure that all residents are treated fairly, and with kindness, dignity, and respect. Ensure that a written record is made of any resident's complaints or grievances with a full accounting of any actions taken and the current status of the complaint.

Working Conditions :

  • Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses' stations, resident rooms, etc.).

  • Moves intermittently during working hours.

  • Is subject to frequent interruptions.

  • Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.

  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.

  • Communicates with the medical staff, nursing personnel, and other department directors.

  • Works beyond normal working hours and on weekends and holidays when necessary.

  • Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).

  • Is involved in community/civic health matters/projects as appropriate.

  • Attends and participates in continuing educational programs.

  • Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.

  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and AIDS and Hepatitis B viruses.

  • May be subject to the handling of and exposure to hazardous chemicals.

  • Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents' needs.

  • Multi facility travel required

  • Must be able to travel by air and automobile with overnight stays.

Requirements for Traveling MDS Coordinator : Education

Must possess a valid license as a Registered Nurse/Licensed Practical Nurse in the state(s) of practice.

Experience

  • Must have, as a minimum, three (3) years of nursing experience in a setting serving the same age or type of resident population served by this facility.

  • Preferred to have two (2) years of experience as a MDS Coordinator.

Specific Requirements

  • Must possess a current, unencumbered, active license to practice as a Registered Nurse/Licensed Practical Nurse in this/these state(s).

  • Must be able to read, write, speak, and understand the English language.

  • Must possess excellent communication and interpersonal skills.

  • Must demonstrate the knowledge and skills necessary to prove care appropriate to the age-related needs of the residents served.

  • Must be a supportive team member, contribute to and be an example of teamwork and team concept.

  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities

Salary: $17.00 per hour, including up to a 40% rent concession in one of our communities should there be an available vacancy.
