Job Description & Requirements Occupational Therapist

• Outpatient - (OT) StartDate: 2/24/2025 Available Shifts: 8 D Pay Rate: $1725.92 - $1891.16 Occupational Therapist

• Outpatient needed asap for 13 weeks M-F 8am-4:30pm

• Some shifts may be shorter COVID-19 vaccination is recommended but not required. Flu vaccination will be required October through April.
  
  Flu exemption applications must be received and approved by Phoenix Children’s prior to starting.
  
  Anyone not vaccinated for either Flu or COVID-19 will be required to sign Declination & Masking Protocol agreements and must mask during the full respiratory virus season. Graduation from an accredited Occupational Therapy program Experience Requirements: 1 year Pediatric work experience (school experience will not be considered) 1 year outpatient experience.
  
  Certification/License/Registry Requirements: Certification with the National Board for Certification in Occupational Therapy (NBCOT) prior to start date State of Arizona Occupational Therapist license prior to start date Current AZ Department of Public Safety Fingerprint Clearance Card prior to start date Current BLS certification from the American Heart Association prior to start date This assignment will be at our Avondale Campus, 1665 N. Avondale Blvd., Avondale, Arizona 85392.
  
  Required Qualifications Occupational Therapist, Outpatient Licenses: OT-AZ Certifications: BLS-AHA(Copy needed) AND Occupational Therapist, Pedi SSN Required DOB Required Work History:
  
  Should not be more than 30 days of gap in last 1 year COVID-19 vaccination is recommended but not required. Flu vaccination will be required October through April.
