• 5x8 - 08:00AM - 04:00PM - 40 Hours Guaranteed Hours: 40 Hours per Week Experience: Minimum of 1 year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health. License: Current license to practice nursing in the desired state of employment Certifications: BLS (American Heart Association) Must-Have:

• Current automobile insurance and valid driver's license

• Ability to effectively operate nursing assessment tools, computers, facsimile equipment, copier, and cell phone/beeper

• Transportation for work purposes Job Description: The Registered Nurse (RN) will be responsible for providing direct patient care services to hospice patients and supportive care to patients' families. Responsibilities include managing an interdisciplinary group with Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality and case managing all assigned patient clinical records.

• Assure a high quality of hospice care is provided and documented according to the Plan of Care.

• Perform initial and ongoing assessments of patients.

• Help develop and implement the Hospice Plan of Care prescribed by the physician.

• Collaborate with the interdisciplinary group for comprehensive service delivery including physical, psychosocial, educational, emotional, and spiritual services as well as bereavement support.

• Report any changes in the patient's condition to both the Attending and Hospice Physicians.

• Maintain accurate and updated medical records, including interdisciplinary notes, Plans of Care, and assessments.

• Practice infection control measures and standard precautions.

• Monitor staff scheduling.

• Participate in the Quality Assessment and Performance Improvement Program.

• Be part of the on-call agency schedule as needed.

• Consistently promote the company's core values.

• Complete required compliance annual training.

• Attend staff meetings.

• Maintain a high level of knowledge on managing pain and symptoms during the dying process.

• Make routine supervisory visits with other team members at least monthly.

• Review and follow up on patient clinical records to ensure quality services, standards of practice, and compliance with policies, procedures, and regulatory requirements.

• Maintain primary control and professional management of each patient, acting as the primary liaison between physician, patient/family, and hospice team.

• Educate the patient, family, or caregivers in relation to identified Plan of Care needs.

• Evaluate patient/family care based on nursing needs. Schedule Notes:

• One position will involve on-call weekends from Friday 5am to Monday 8am, guaranteed 40 hours per week.
