• Manage and coordinate hospice care for patients within designated territories.

• Provide compassionate and effective care to patients and their families.

• Collaborate with interdisciplinary teams to develop and implement individualized care plans.

• Conduct routine visits to assess patient conditions and adjust care plans as necessary.

• Educate patients and families regarding care plans and hospice services.

• Maintain accurate and timely documentation of patient care activities.

• Participate in regular team meetings to ensure holistic and coordinated patient care.
