Travel Nurse RN - Case Manager In Santa Rosa, CA

Weekly Pay Range - $2345 To $2605 Job Title: Registered Nurse Job Specialty: Case Manager Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not specified Experience: 2 years working in Utilization Management (admissions, utilization review, and/or discharge planning) License: CA RN Licensure Certifications: BLS (AHA) Must-Have: - CCM-Certified Case Manager preferred

• Epic charting experience preferred Job Description: - Coordinate admissions and perform concurrent and retrospective utilization reviews.

• Facilitate discharge planning and provide coordination for complex discharge needs.

• Make referrals to appropriate support services when necessary.

• Work the shift: 5 x 8 Days, 08:00 am

• 5:00 pm, with variable weekends and expectation to work every other weekend.

• No on-call requirement.

• Attend orientation for 16 hours.

• Must be able to provide a background check that notes any background hits or DUIs prior to submission.

• Ensure all certifications are in hand and up-to-date at time of application.
