• Alternate between different units providing comprehensive patient care.

• Engage in complex discharge planning to ensure continuity of patient care.

• Utilize EMR systems like Meditech and Midas for efficient documentation and patient tracking.

• Collaborate with a team consisting of charge nurses, RNs, CNAs, and social work case managers.

• Maintain patient ratios of at least 25:1 for case management.

• Participate in a rotating schedule, working every other weekend and holidays.

• Promote a collaborative and positive work environment, leveraging a highly experienced staff.
