Travel Nurse RN - Case Manager In Carmichael, CA

Weekly Pay Range - $2954 To $3282 Job Title: Registered Nurse (RN) Job Specialty: Care Coordination/Case Management Job Duration: 13 weeks Shift: 8-hour Days, Every Other Weekend, First to Float Guaranteed Hours: 40 hours per week Experience: Minimum 2 years License: Required Certifications: BLS Must-Have: Prior experience with specified health system or facility Job Description: - Manage care coordination and case management, including utilization review and discharge planning.

• Provide initial assessments and discharge planning using Interqual, with training available for candidates.

• Collaborate with all payers, manage cases involving uninsured and homeless patients.

• See and consult with 15-20 patients per day.

• Work across all areas of the hospital, excluding outpatient and home care settings.

• Participate in a 5-day care coordination and new employee orientation, with additional unit orientation based on individual needs.

• Exhibit strong skills in collaboration, teamwork, effective communication, and ability to reprioritize tasks throughout the day.

• Work 2 out of the 3 major holidays, with holiday time off considered but not guaranteed.

• Wear business casual attire as uniform.

• Shifts are between 8 AM and 5 PM, with at least one full weekend per month required, depending on staffing levels.

• Engage in both discharge planning and utilization management within the hospital's acute care setting.

• Work with Cerner charting system; previous experience preferred. Note: Hospital may cancel up to three times per contract, with specific policies for cancellations and shift rescheduling.
