Summary: Assists the health care team in implementing the plan of care.

Responsibilities:
Assists with preparation for patient's discharge (e.g., check belongings, get wheelchair & escort patient out). Seeks assistance as needed from appropriate resources and recognizes the need for emergency intervention.

Promotes learning for colleagues and self, functions as a preceptor. Proactively identifies risk to patient safety and identifies opportunities to reduce medical/health care errors (i.e., appropriate reporting of errors/incidents). Identifies ethical issues and reports them to the RN. Reports in a timely manner to the primary RN the patient's complaint of pain. ; Transports patients to & from different areas of hospital safely and efficiently.

Assists the staff in trasferring patients to and from stretchers, wheelchairs, etc. Maintains and restocks supplies & equipment (e.g., gloves, oxygen tanks, wheelchairs, stretchers, etc.) and precaution carts. Reports malfunctioning equipment.

Supports hourly rounding in the department. Incorporates the 4Ps during rounding - Pain, Potty, Positioning, Personal Possessions. A ssists in maintaining an orderly, clutter-free and clean environment.

Aheres to Infection Control policies. Deliver and pick up of supplies, meds, labs, etc. to and from other departments.

Qualifications:
BLS required. HS Diploma or equivalent required as of July 26, 2019.

Physical Requirements:
Exerts up to 20 to 50 pounds of force occasionally, and/or up to 25 pounds of force frequently, and/or a negligible up to 10 pounds of force constantly to move objects. Frequently reaches (extending hands and arms in any direction), and handles (seizing, holding, grasping, turning, or working with hands). Occasionally stoops (bending the body downward and forward by bending the spine at the waist), and kneels (bending the legs at the knee to come to rest on knee or knees).

The primary focus is patients requiring telemetry monitoring, recovery post cardiac catheterization, and monitoring patients with cardiac dysrythmia, chest pain, CHF, dig toxicity, stroke and TIA. Responsibilities: * Provides patient care based on standards of care, standards of practice, protocols and established policies and procedures. * Collaboratively plans and prepares patient/family for discharge incorporating support systems/resources as necessary. * Appropriately prioritizes patient care, (e.g. seeks assistance as needed from appropriate resources, reassigns personnel as necessary, recognizes need for emergency intervention, etc.) * Provides thorough, timely and accurate documentation of all pertinent data, therapeutic interventions and patient responses including assessment, care plan, patient goals and interventions according to established standards. * Proactively identifies risk to patient safety and identifies opportunities to reduce medical/health care errors (i.e., appropriate reporting of errors/incidents). * Safely administers medications and intravenous therapy in accordance with physician's orders, protocols, policies and procedures. * Assess patient pain level upon admission and as necessary, utilizing the pain scale, (e.g. numerical, FACES, and functional goals.) * Reassesses effectiveness of pain control and notifies physicians as needed, as well as, documents assessment and patients' response to medication. * Readily accepts accountability for designated patient and effectively hands off patient as required. * Appropriately delegates and supervises nursing interventions performed by others. Qualifications: * BSN preferred. * Current MA RN License. * BCLS required * Minimum 2 years hospital experience preferred * Must be ACLS certified by the end of the nursing orientation perirod.. * Minimum of 2 years of telemetry experience preferred.
