Short Description

• Coordinates care for cancer patients and their families in collaboration with an attending physician and other members of the MD Anderson Cancer Center at Cooper team to provide expert individualized care.

• Assist patients and families with adjustment to illness, role changes, and education regarding diagnosis and treatment as appropriate.

• Facilitates compliance with plans of care

Experience Required

3 - 5 years health care experience preferred

Education Requirements

Requires a Master Degree in Social Work (MSW)/ or BSWor a licensed RN, BSN preferred

License/Certification Requirements

Current NJ SW License, LCSW Preferred.

Current NJ RN License

ACM (American Case Management Association) preferred

CCMC (Commission for Case Manager Certification) preferred
