Short Description

Provides psychosocial assessments, crisis intervention, resources referrals,

to facilitate discharge plans, and/or adjustment to illness, and

complex discharge planning for patients and their families.

Formulates the discharge plan with patient, families and the care team based upon a needs assessment.

Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community based programs.

Coordinates care authorization process with insurers

Coordinates specific details of patient's hospitalization with Utilization Management department to ensure appropriate admission status

Special Requirements

Scheduled Days / Hours: M-F days 8:00 - 4:30


Requires a Master Degree in Social Work (MSW) or a licensed RN, BSN preferred

3 - 5 years health care experience

Required License/Certifications/ Registration:

Current NJ SW License, LCSW Preferred.


Current NJ RN License

ACM (American Case Management Association) preffered

CCMC (Commission for Case Manager Certification) preffered

General Summary: The ITSM - Incident and Problem specialist will work closely with the ITSM Incident and Problem lead to manage, govern, train, and guide the IT organization in the ITSM processes. They will look across the IT organization for opportunities to bring or increase value with these ITSM processes. The key objectives for this role are: * Manage and oversee the efficiency and effectiveness of the Incident and Problem processes in the ITSM tool. * Manage current usage and adoption of the ITSM modules - Incident and Problem processes. * Look for opportunities to refine and expand the Incident process to improve the customer experience within IT and the organization * Look for opportunities to refine and expand the Problem process to improve overall IT Service within the Organization * Be a champion for the ITSM processes across IT and entire organization This role requires leadership, people management, technical, organizational and communication skills to manage the ITSM Processes, collaborate with IT and business organizations, interface with the IT organization and communicate effectively with stakeholders, while overseeing the overall design and on-going maintenance of all service transition processes to meet the overall transition needs of the business. Primary Responsibilities: * Incident Process * Ensure incidents are processed and resolved in a timely manner * Ensure the groups, roles and individuals are set up within the Incident module * Responsible for the development and delivery of the training for the Incident Process * Follow up on Customer Surveys to ensure the customer is satisfied * Design run and schedule reports for the IT and Customer teams. * Look for opportunities within the tool and process for improvement and manage them into production * Problem Process * Identify re-occurring or high priority Incidents that should be problems * Follow up on Problems records and corresponding tasks to ensure timely completion * Ensure the groups, roles and individuals are set up within the Problem module * Schedule and run Root Cause Analysis meeting * Responsible for the development and delivery of the training for the Problem Process * Design run and schedule reports for the IT and Customer teams. * Look for opportunities within the tool and process for improvement and manage them into production * Rotate as key leader during Major Incidents. Primarily serve as Major Incident Commander, Scribe or Impact Lead but not limited to these roles. Minimum Requirements: * Bachelor's degree in Information Technology or related field required * 2 - 5 years relevant IT experience required * Previous experience with Service Now tool * Previous experience as with Incident and Problem process as a user, contributor or owner * Experience and working knowledge of ITSM and ITIL framework, ITIL certification is a plus * Proven competency in executing projects, on time and within budget, with business and IT personnel * Ability to work and execute with global teams and diverse cultures * Strong written and verbal communication skills, including presentation skills * Develop training material and present to in person and online audiences * Leadership Skills The Company is committed to providing equal opportunity for employees and applicants in all aspects of the employment relationship, without regard to race, color, sex, sexual orientation, gender identity, national origin, citizenship, marital status, veteran status, disability, age, religion or any other classification protected by law.
