Transition Specialist - Part Time (24 hrs a week) - Northfield Campus

Standard Summary:

Responsible for transitioning students who attended the Hyde Park Day School back to schools in their home community. Provide services (e.g., consultation with teachers, coordinating counseling, tutoring and provides families with support through the IEP process) to students who have transitioned.

Plan, coordinate, and supervise all graduation activities. Contribute follow-up research studies on students who have transitioned. Represent Hyde Park Day schools and professionals in the community. This position is based at the Northfield Campus.

Essential Functions:

  • Placement of students who are transitioning to school in their home communities. Utilize multiple means of communication with parents, invites parents to assist with the placement of their child.

    Read files and meet with students' teachers, tutors, psychologists, psychiatrists, and other therapists, as appropriate. Observe, meet with the students, and do testing, as appropriate. Meet with administrators and teachers at the schools.

    Seeks assistance, advice and guidance as necessary from colleagues. Exhibit professional ethical behavior toward coworkers. Sincerely care about the students and demonstrate sensitivity to student's needs.

    Participate in activities to communicate with and include the community at large when appropriate. Display a positive attitude that shows excitement and enjoyment of students. Is approachable so students and parents are comfortable asking for help.

    Ability to read, write, and speak clearly and accurately in the English language at a professional level. Consistently reliable and dependable.

  • Provide services (e.g., consultation with teachers, coordinating counseling and tutoring) to students who have transitioned). Coordinate other services as needed.

  • Ongoing professional growth, be knowledgeable and stay current in core curriculum disciplines that directly relate to pupil learning. Maintains professional competence through such activities as reviewing research journals, attending in-service workshops and classes, and sharing that information with other educators as appropriate. Be reflective, practice self-assessment, and devises plans for professional improvement.

  • Plan, coordinate, and supervise all graduation activities (e.g., graduation, yearbook).

  • Provide families with support through IEP process.

Education/Experience Minimum Requirements:

Bachelor's degree or higher in learning disabilities, special education, or social work required.

Master's degree preferred.

At least three years of experience working with children with diagnosed learning disabilities preferred.

Skills and Knowledge Requirements:

Experience with appropriate educational placement of students with learning disabilities required.

Familiarity with the public and private schools in the Northfield and Chicago Metropolitan area preferred.
