The University of Iowa Hospitals and Clinics, Center for Disabilities and Development (CDD) is to hire a 100% Sr. Behavioral Health Consultant (Transition Specialist). This position is specified term and funded through March 31, 2025 with a strong possibility of being extended. Successful candidates must be able to maintain a home office and ability to travel extensively throughout the state of Iowa. We are looking to hire a candidate in Central or Eastern, IA.

This position is eligible for remote work within Iowa and will require a work arrangement form to be completed upon the start of your employment. Per policy, work arrangements will be reviewed annually, and must comply with the remote work program and related policies and employee travel policy when working at a remote location.

Position Summary:

The Transition Specialist facilitates the process for individuals transitioning from a facility to a home or community-based setting of their choice with provision of supports and services that meet those individual's needs and preferences. Transition services include individual assessments, resource counseling, care coordination and processing of typical and non-typical Medicaid authorized services needed to support the participant's needs. Extensive travel throughout Iowa will be required. State vehicle provided.

Position Responsibilities:

  • Serves as initial point of contact for consumers and family members/guardians interested in transition.

  • Prescreens individuals interested in transitioning from a qualifying facility to an integrated community setting.

  • Assist in developing and implementing strategic action plans.

  • Contributes to the development and implementation of sustainability projects to support and enhance Iowa's community living capacity for the served population.

  • Provide technical assistance to partner entities.

  • Initiates and oversees individual assessments for clients, including coordinating Level of Care assessments (completed initially and periodically). Initiates requests for other assessments and determines if additional assessments would be appropriate and complete if necessary.

  • Provides care coordination, case management and transition monitoring for the 365-day demonstration period for each client to assure a successful, seamless transition.

  • Coordinates care with all members of the care team; conducts regular in-person visits with clients and identifies situations in which client would benefit from a consultation with the employment or behavioral specialist.

  • Provides options counseling.

  • Completes State of Iowa Options Counseling and reviews Medicaid and non-Medicaid resources and what might be available for client.

  • Coordinates client transitions:

  • Researches service options and works with clients to make determination about and secure service providers.

  • Serves as point of contact for client and service providers.

  • Works with client to obtain housing, household items, daily living and durable medical goods.

  • Completes all related Department of Human Services and UI paperwork (transportation release, media releases) related to the transition.

  • Coordinates services and provides frequent updates with client's Managed Care Organization (MCO); program that will continue funding past the client's initial 365 days with MFP.

  • Assist Program Manager with decisions related to hiring MFP staff including participating in interview and selection processes.

  • May assist Program Manager in training new students and staff in their roles and responsibilities for the MFP program.

  • Manages financial resources for transitioning clients.

  • Completes funding authorization request and submits for approval.

  • Processes payments for typical Medicaid (supported community living services, respite, transportation, employment rehabilitation) and non-typical Medicaid authorized services (household set up items, assistive devices, environmental modifications and durable medical goods).

  • Manages UI procurement card and cash advance funds.

  • Uses p-card and cash advance funds to make durable good purchases for clients.

  • Manages complex reconciliation of receipts and p-card statements to meet monthly deadline.

  • Participate in outreach activities to raise and maintain awareness of the option to transition consumers currently living in facilities to the community of their choice. Work to build provider interest.

  • Completes required documentation to include maintaining client files, monthly service documentation, contributing to grant reporting, and other related tasks.

  • Addresses customer service concerns.

The CDD is Iowa's University Center for Excellence on Disabilities and is also home for Iowa's LEND (Leadership Education in Neurodevelopmental and Related Disabilities) program and has provided outstanding interdisciplinary training in neurodevelopmental disabilities for over two decades.

Required Qualifications:

  • Bachelor's degree or equivalent combination of education and responsibility

  • 1-3 years of experience as a case manager for people with disabilities or in a similar human services position

  • Demonstrated knowledge about Community Home Based Services systems and housing and service options

  • Demonstrated knowledge about Intellectual Disability/Brain Injury and related conditions and knowledge of available resources for information and assistance

  • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member

  • Demonstrated organizational skills and ability to manage multiple tasks and complete tasks in a timely manner

  • Demonstrated proficiency in computer software applications, including word processing and spreadsheet software

  • Ability to travel extensively within the State of Iowa and hold a valid Driver's License, plus the ability to meet the University of Iowa Fleet Safety Program driving standards (University of Iowa Operations Manual Part V, Chapter 19) and ability to travel extensively within an assigned service region

  • Ability to maintain a home office and meet technology requirements to conduct business and video conferences using a home internet connection

  • Ability to work a flexible schedule

Desired Qualifications:

  • Master's Degree in a related field

  • Previous experience with a consumer-focused program; and/or experience empowering consumers and caregivers in making informed choices and finding solutions to support consumers through this process

  • Previous experience using mediation skills or other techniques to assist consumers in prioritizing needs and developing an options plan

  • Previous experience building collaborative working arrangements with other human services programs, families and communities to create a circle of support for the consumer. Skills in building rapport and developing relationships

  • Knowledge of University of Iowa and UI Healthcare policies, procedures and regulations is desirable
