Additional Information:
Assist adolescent and young adult members to receive early, holistic, comprehensive, and culturally and linguistically competent transition planning to prepare them for service and benefit changes that will occur following their 21st birthday and the transition from a pediatric to adult health care model.
Education:
Requires a Master of Social Work degree.
Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions.
Knowledge of STAR+PLUS benefits/services, community resources, LTSS, waiver programs, educational IEPs, vocational programs desirable.
Bi-lingual (English/Spanish) Preferred. No testing requirement.
Required skills: Relationship-building, Effective written and verbal communication, Critical thinking and problem-solving. Ability to plan and organize effectively Proficient in use of computer applications
