Position Summary

The Transition Services Manager provides leadership for an innovative, skilled multi-disciplinary team dedicated to engaging, assessing, and supporting culturally diverse homeless young people ages 16-25. This position provides supports the Youth Department, including providing clinical staff supervision as well as program and system coordination. The position is responsible for supporting the effective utilization of harm reduction, positive youth development, motivational interviewing, and trauma-informed care throughout the Youth Department.

The Transition Services Manager is responsible for the design, development, evaluation and coordination the following programs:

  • Assertive Engagement Services (case management),

  • Transitional Housing Case Management,

  • Parent Support Program,

  • Community Housing Navigation, and

  • Other projects related to transition support services.

The Transition Services Manager is responsible to:

  • Hire, train, and supervise program staff. This includes all program coordinators, Case Managers, Housing Case Managers, Parent Support Specialists, Health Navigators, and interns/students/volunteers.

  • Provide clinical supervision.

  • Coordinate effectively with Volunteer Program for placement and oversight of student/interns/volunteers.

  • Provide support for program services.

  • Support the effective utilization of harm reduction, positive youth development, motivational interviewing, and trauma-informed care throughout the Youth Department.

  • Work with staff to develop a schedule of youth support and staffing that covers 7 days a week.

  • Ensure adequate staffing of programs and program activities.

  • Ensure quality provision of services.

  • Develop and sustain effective working relationships with partner and other community organizations.

  • Integrate the program offerings with other Youth Department and agency services, and community resources when possible.

  • Serve as a member of the Youth Service Department and agency coordination teams to integrate programs and coordinate services.

  • Provide crisis response and support for other crisis responders.

  • Provide after hours support to the Youth Department on a rotating basis.

  • Oversee smooth function of the material and system operations of the program.

  • Monitor budgets and complete reporting in a timely manner or delegate as appropriate.

The long-term goal of the Transition Services Manager is to help youth access the resources, build the skills, and access the other supports needed to progress toward self-sufficiency and community integration. Included in this can be supporting youth to be successful in the common steps toward self-sufficiency: employment, education, and stable housing.

Essential Duties

Supervision and Training

  • Provide adequate orientation and training for all program staff, interns/students, and volunteers.

  • Assess the training needs of program staff and arrange to meet these needs in collaboration with the Youth Department Director.

  • Establish and facilitate effective communication with staff. This includes being a conduit of information between program staff and the Department Director, as well as updating program staff on resource, referral and other information they need to perform their job responsibilities.

  • Provide clinical and program supervision to all program staff, interns/students, and volunteers.

  • Build an effective and supportive team culture.

  • Facilitate a self-managing team with effective and empowered teamwork among all program staff.

  • Assist in generating and updating staff job descriptions.

  • Provide performance feedback to program staff throughout the year and at the probationary and annual performance reviews.

  • Ensure process for effective facilitation of team meetings, including staffing of clients.

  • Develop and implement processes to recognize staff for their accomplishments and to support them in their hard work and job stresses.

  • Develop and conduct trainings relevant to staff professional develop, including assisting in facilitation of established Outside In trainings offered.

Scheduling

  • Ensure adequate schedule of team coverage, including case managers working scheduled 7 days a week to meet the needs of youth.

  • Work with team to ensure basic coverage when staff out for planned or unplanned absence.

Program Planning

  • Plan services as well as improvements for programs to meet the desired outcomes stated in contracts.

  • Evaluate, on a regular basis (at least quarterly), program service effectiveness at achieving contracted outcomes.

  • Ensure that program supplies, including client assistance supplies and other equipment/ materials, are available to staff as needed.

  • Monitor and disburse, as needed, resources accessed through the program.

  • Work with the team of program staff to evaluate team function and implement program changes.

  • Participate in both overall agency planning, Continuum planning, and community planning, including coordination and integration of program elements.

Policy and Procedure Development and Update

  • Assist in developing, updating, and maintaining program expectations and operational policies and procedures for the program.

  • Review safety practices in the program with the Youth Department Director, other members of the Youth Department Leadership Team, and the Agency's Safety Committee to ensure that safety standards are met and maintained.

Continuum and Community (External) Coordination

  • Develop and maintain systems for receiving Continuum referrals/screenings, Assessment completion, tracking, assignment, and follow through.

  • Participate in community/partner meetings, including regularly scheduled Continuum team meetings and work groups, to represent Outside In.

  • Represent Outside In professionally at meetings of community service providers and funders.

  • Promote and maintain favorable working relationships with outside agencies to ensure coordination of services. Specifically, coordinate with the Access Center regarding referrals, assignments, and closures; and all partner agencies regarding case management and housing capacity.

  • Coordinate with community resources to remove barriers and enhance youth's opportunities for success.

  • Represent Outside In, the program, and client needs at interagency meetings.

  • Keep informed of community services and youth issues and oversee that updated information is available to all staff.

  • Work with staff and community members to integrate program services into the community and its resources.

Agency and Department (Internal) Coordination & Integration

  • Support program integration with the other Youth Department Programs.

  • Participate in regular meetings including Youth Department Leader Meetings, weekly Program meetings, and quarterly Department meetings.

  • Assist in facilitation of Department and Program Meetings.

  • Support youth Plans as created with various staff throughout the Youth Department.

  • Support program staff in maintaining a healthy program environment and building youth's life skills.

  • Build and support effective, coordinated interdisciplinary teams including the 'case management' and the 'housing' teams.

  • Work with Engagement Services Manager to ensure a smooth process of monitoring the waiting list for case management (Assertive Engagement).

  • Make timely and balanced assignments to Case Managers, taking into consideration information from staff.

  • Assist in coordinating the teamwork of the Housing Case Managers and Housing Specialist staff.

  • Share information about interventions with youth's Case Manager and Contact Person when appropriate; participate in case planning to manage future crises when possible.

  • Ensure integration of MH and A/D services within programs.

  • Ensure effective referral of youth and coordination of staff efforts with ERC/Urban Ed.

  • Coordinate with other Youth Department programs and the Clinic to support smooth coordination of services, enhance youth opportunities, and remove barriers to youth success.

  • Work with agency staff to coordinate and integrate program services throughout the agency.

Crisis Counseling

  • Provide crisis counseling services to clients as needed.

  • Share information about each intervention with the client's Case Manager and/or Contact Person when appropriate, and participate in case planning to manage future crises when possible.

  • Utilize harm reduction, motivational interviewing/stages of change, trauma informed care, and youth development philosophies in problem solving to help clients identify healthier options.

  • Coordinate with Outside In Mental Health Resources, Project Respond, Crisis Triage and other services as needed.

  • Provide after hours crisis support to the Youth Department on a rotating basis.

Support Youth Participants

  • Build positive relationships with youth through role modeling.

  • Be available on site to assist with youth participants as needed, which can include evening and weekend hours to accommodate program schedules.

  • Provide back-up support to staff in interacting with youth and addressing youth needs and concerns.

  • Participate in crisis planning and crisis intervention, which may include after hours contact on an as needed basis.

  • Provide support for youth to accomplish the goals identified in their Individual Service and Support Plan or other individual goal plans.

  • Assist staff in monitoring youth compliance with program agreements.

  • Initiate and monitor participant consequences, including exclusions, as needed.

  • Assist in re-entry meetings with youth as needed.

  • Support youth in developing constructive use of time through community involvement, healthy recreation, skill building opportunities, etc.

Youth Involvement

  • Involve and empower youth participants in generating community within the Youth Department.

  • Support youth generated activities and governance.

  • Respond in a timely and respectful manner to all youth requests.

Data, Reporting, and Service Documentation

  • Maintain accurate and timely documentation of all services and interventions.

  • Work with staff to ensure that services are documented to meet all program and funder requirements.

  • Complete all required service and data reports in a thorough and timely manner.

Quality Assurance and Oversight Services

  • Observe Youth Department Philosophy and Policies.

  • Develop and maintain internal administrative policies, procedures, and records to ensure quality services provided to youth.

  • Review and sign off on Assessments, Plans, and other client documents to ensure documentation meets agency and funder standards.

  • Monitor progress of Youth Department to ensure contractually agreed upon service levels, goals and objectives.

  • Ensures service records to meet State ORS and ISSR standards.

  • Coordinates regular chart reviews and provides supportive feedback to staff to meet documentation standards.

  • Provides training to Case Managers and other staff to meet documentation standards.

Program Operation & Budget Management

  • Assist in providing material and operational support in programs.

  • Ensure that program supplies, including client assistance supplies, are sought and obtained in the most cost-effective manner.

  • Ensure program supply inventory is stored in an organized fashion.

  • Coordinate with the Support Services Director and maintenance personnel to ensure smooth operation of the program, including maintenance and basic upkeep of the program spaces.

  • Encourage youth involvement in maintaining the program space as much as possible.

  • Monitor program budgets.

  • Approve requested expenditures with available budget.

  • Assist in annual budget planning process.

Education / Experience

  • Proof of COVID-19 vaccination status or approved medical or religious exception on file is required.

  • MSW/MA Counseling; LCSW licensure preferred but not required.

  • At least 2 years project coordination and supervision experience;

  • At least 5 years of experience in mental health and/or substance use treatment, homeless populations, and at-risk transition age youth / adolescents;

  • Excellent communication skills;

  • Excellent crisis intervention skills;

  • Knowledge and experience working with community resources.

  • Knowledge of clinical and housing documentation requirements.

  • Knowledge and understanding of adolescent development, Trauma Informed Care, Stages of Change Theory, harm reduction and Motivational Interviewing.

  • Ability to write and supervise the implementation of an Individual Service Plan,

  • Strong organizational skills, attention to detail, and focus on data collection.

  • Budget management skills.

  • Ability to work with a diverse population and interact effectively with different types of people.

  • Excellent communication and conflict resolution skills.

  • Ability to work both independently and as a member of a team.

Working Conditions

This job includes working in a standard office environment, drop-in program, and kitchen, as well as a variety of community locations to support effective operations of the Day Program. This means that the employee will meet in a variety of partner service locations and in other community. The employee will may be responsible for transporting participants in a rented vehicle, when needed.

Physical Requirements

This job requires regular sitting, standing, walking, talking, seeing and hearing. The employee frequently is required to use hands and fingers to type, answer phones and forward calls. The employee is occasionally required to stand and walk. The employee must frequently lift and/or move up to 20 pounds.
