This is a full time position with 8-hour day shifts at Scripps Mercy Hospital San Diego. Includes $4,000 retention incentive and $3,000 relocation assistance to eligible new hires.

Pharmacy practice at Scripps Mercy Hospital San Diego is the most advanced in the county, boasting significant pharmacist practice authorities and exceptional pharmacy-provider working relationships. As a teaching hospital, Mercy San Diego values the investment in the future of pharmacy, hosting pharmacy students and residents every year. The Medication Therapy Management (MTM) service is a high volume, house-wide, consult-based service. The ideal candidate will possess a background in medication therapy management and will need to have a commitment to patient well-being, possess excellent critical thinking and communication skills.
Responsibilities include:
Ensures the safe, effective, and cost-efficient management of medication therapy for patients, conducting comprehensive medication reviews, and collaborating with healthcare providers to ensure the highest quality of patient care.
Manage transitions of care from inpatient to outpatient
Perform admission and discharge medication reconciliation as appropriate
Round on inpatient teaching medical team
Precept pharmacy students and residents

In order to remain eligible for your retention incentive the following criteria must be met:
Must remain in original hired FTE Status and Shift (if specified in offer letter)
Must remain in original department/specialty
Must remain in original Job Title
Transfers to other locations will be reviewed on a case-by-case basis and may result in forfeiting remaining incentive bonus unless specifically noted in your offer letter.

Required Education/Experience/Specialized Skills:
Required Certification/Registration:
California Pharmacist License
AHA BLS for the health care provider and ACLS certifications required prior to being trained to respond to codes.
Board of Pharmacy Specialties (BPS) or other nationally accredited certification required within 2 years' of hire.
Preferred Education/Experience/Specialized Skills/Certification:
Experience in Medication Therapy Management, proficiency in utilizing MTM platforms and tools
Experience in a multi-site health system pharmacy enterprise
Teaching certificate and/or experience precepting students and residents
