At LSI, we are dedicated to wrapping compassionate care around the children and families we are privileged to serve. Our therapists are central to our mission and serve as expert advisors to LSI's early childhood, behavioral health, and family safety teams.

We are seeking a full-time Therapist to provide therapy and counseling to couples, families, adults, and children in Denison, Iowa.

In this role, you will:

  • Provide therapy and counseling assessments in a community-based setting

  • Have the potential to see clients in a school-based setting

  • Serve a diverse population of children, adults, couples, and families

  • Develop individual treatment plans to fully address each client's various behavioral and mental health care needs

  • Manage case plan oversight and re-evaluate as necessary to build the best possible outcomes for our clients

  • Have the ability to receive free clinical licensure supervision from one of our Clinical Supervisors

Qualifications

  • A Master's in Social Work or an approved related field (LMFT, LMHC)

  • Must be licensed in the state of Iowa (temporary and provisionally licensed applicants are encouraged to apply!)

  • A valid driver's license, good driving record, and auto insurance

  • Must be able to pass an extensive background check, including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry, and a criminal history check

The OT delivers therapy intervention and treatment directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.</p><ul><li><p>Makes visits to the homes of patients requiring home health occupational therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient..</p></li><li><p>Evaluates and assesses the patient's level of functioning by applying appropriate tests.</p></li><li><p>Develops specific occupational therapy procedures as well as functional activities in accordance with written physician plan of treatment and patient evaluation.</p></li><li><p>Develops and administers plan of treatment including environmental modifications.</p></li><li><p>Oversees use of treatment modalities, prosthetic devices and therapeutic appliances.</p></li><li><p>Ensures quality and safe delivery of services to patients according to the care plan.</p></li><li><p>Develops and updates patient's discharge plan on every visit ensuring patient and caregiver education and training is facilitating timely discharge and measureable goals and outcomes are attainable, realistic and up-to-date.</p></li><li><p>Maintains competence in OASIS data collection and documentation and submits all initial and corrected assessments according to CMS guidance and agency policies and procedures.</p></li></ul><p>Qualifications</p><ul><li><p>Licensed or otherwise regulated, if applicable, as an Occupational Therapist by the state in which practicing, unless licensure does not apply in the practicing state.</p></li><li><p>Graduate of an occupational Therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE</p></li><li><p>Eligible to take, or has successfully completed the entry-level certification examination for Occupational Therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc.
